贲门周围血管离断术治疗门脉高压症的近远期疗效

摘　　要：报告１９７２年５月～１９９２年１２月间．应用贲门周围血管离断术治疗各种类型门脉高压症２４７例．急诊手术３５例，择期治疗性手术１７６例及预防性手术３６例，近期止血率为９８．４％．手术总死亡率为３．６％，其中急诊手术为１７．１％，择期手术为１．３６％．远期再出血率５ａ为６．３％、１０ａ为１２．２％、１５ａ为１８．６％及２０ａ无再出血．肝性脑病发生率５ａ为２．４％、１０ａ为４．２％、１５ａ为５．５％及２０ａ为６．５％．肝功能改善者为７２．７％及脾功能亢进改善者为８９．１％．Between 5. 1967 and 12.1992, the author performed pericardiac devascularization upon 247 patients who suffer from a variety of kinds portal hypertension. 35 of the operation were done emergently, 176 were done electively and 36were done prophylactically. The short-termcontrol bleeding rate was 98. 4% the total mortality rate was 3. 6%. The mortality rate of emergency operation was 17. 1 %,and the mortality of elective operation was 1. 36%. The long-term rebleeding rate was 6. 3% in 5a, 12.2% in 10a, 18.6% in 15 a and 0% in 20 a. The hepatic encephalopathy rate was 2.4% in 5 a, 4. 2% in 10a, 5.5% in 15 a and 6. 5% in 20 a. The 72. 7%of patients show improved liver function.and 89.1% of them show improved hypersplenism.
