雷公藤醋酸乙酯提取物在肾移植中应用的实验研究

摘　　要：本研究利用雷公藤醋酸乙酯（ＴＷＨ）提取物在同种大鼠肾移植中的应用，观察其作为免疫抑制剂在肾移植中的抗排斥效能，并使之与常用的免疫抑制剂硫唑嘌呤比较。实验结果表明：ＴＷＨ提取物及硫唑嘌呤均能明显延长大鼠肾移植物的存活时间，两组间受体存活时间无显著差异；ＴＷＨ提取物对受体外周血淋巴细胞化学发光的抑制程度与硫唑嘌呤相当；对外周血Ｔ淋巴细胞的影响前者抑制ＣＤ３、ＣＤ４的增生，增加ＣＤ８数目，后者对ＣＤ３、ＣＤ４及ＣＤ８均抑制；毒副反应观察硫唑嘌呤对肝脏损害及其减少血白细胞总数的作用较ＴＷＨ提取物明显；ＴＷＨ提取物在治疗剂量范围内对受体肾功能无明显影响。结论是：ＴＷＨ提取物在同种肾移植中的总体作用优于免疫抑制剂硫唑嘌呤。The effect of TWH,a traditional Chinese medicine,on renal transplantion inrat,was investigated.The modified model of renal transplantation in rat was established us-ing microsurgery technuque(Wister outbred rat-SD outbred rat).The rats were diveded intothree groups:control group,TWH group and Aza group.The efficiency of immunosup-pression and the side effect of TWH and Aza were evaluated by comparing to each other.Thesurvial time of the recipients in each group was observed.The results showed that:①TWH and Aza both suppressed Ly-CL which repreesents therepresents the early functional activity of the lymphoctye of the recipients.②TWH reducedthe numbers of CD3 and CD4 cells,increased that of CD8 cell; Aza reduced the numbers ofthem all.③The survival time of the recipients in control group was 11.7±1.3 days(x±s),that in TWH group was 18.7±4.7 days,the difference was very significant(P<0.01),thesurvival time of the recepients in Aza group was 18.6±3.7 days(x±s),when comparing tocontrol group it was very significant,too.In the experiment,the efficiency of TWH prolon-ging the mean survival time of the recipients corresponded to that of Aza (P>0.05).④BothTWH and Aza were not enough to inhibit acute renal rejection completely. The histologicalexamination of the renal allografts of both TWH group and Aza group revealed severe renalrejection when the recipients were dying.⑤As Aza,TWH had also some side effect when itwas administered,such as weight loss,impairment of liver cell,but the degree of side effect ofTWH was lower than that of Aza,On the 10th day after transplantation,the renal function ofthe TWH administered recipients was normal ,it revealed that in this dosage the impairmentof kidnev was not obvious.It is conclud that TWH is an effectice immunosuppressant and is believed to be superiorto Aza,but futher study and refine of TWH is necessary.
