腹腔镜下困难胆囊切除术的经验（附391例报告）

摘　　要：应用腹腔镜施行困难的胆囊切除共３９１例，其困难包括广泛的腹腔和Ｃａｌｏｔ三角区粘连、胆囊管及Ｈａｒｔｍａｎ氏囊急慢性嵌顿结石，胆囊萎缩等。本文介绍了作者的经验及术中技术原则，认为：当术者遵守这些技术原则时，腹腔镜下困难胆囊切除仍是安全可行的。A total of 391 cases of difficult laparoscopic cholecystectomy were carried out.Operation difficulties included extensive adhesion in the abdominal cavity and Calot triangle,impacted stone at the Hartman cyst and cystic duct,the shrunker gallbladder.Author's experience and technical principles are introduced.It's concluded that difficult laparoscopic cholecystectomy remain safe if the recommanded technical principles are followed.
