摘  要:采用放免法观测31例肺炎患儿用常规剂量丁胺卡那霉素治疗前、后尿微量蛋白——白蛋白(Alb)、β_2微球蛋白(β_2M)、Tamm-Horsfall糖蛋白(THP)的变化,与24例用青霉素钠盐、氨苄青霉素等非肾毒性抗生素治疗的肺炎患儿尿微量蛋白的变化进行对比。结果两组间主要受肾小球滤过膜影响的Alb在各观察期间含量无明显差异。受肾近曲小管功能影响的β_2M和由远曲小管、Henle氏袢升支上皮细胞分泌的THP均显著高于对照组,但停药后即可逐渐下降。提示常规剂量,疗程使用丁胺卡那霉索对肾小球无损伤,对肾近、远曲小管可有一过性轻微损伤。The microproteinurias (albumin, beta 2-mieroglobulin, Tamm-Horsfall protein) of thirty-One children with pneumonia were studied by radioimmunoassay before and after amikacin treatment. Twenty-four children with pneumonia treated with aq-penicillin or ainpicillin were served as controls. Test results between two groups showed that there was no notable difference in urinary albumin, which influenced by glomerulus. In study group, Beta 2-microglobulin, which was influenced by proximal tubuli fenales contbrti,. was notab!y higher than that in the control group after 3 a:nd 7: days treatment. The Tamm-Horsfall protein secreted by distal tubuli renales contorti and Henle's' tubules, was also higher than that of controls. When amikacin was stopped, the microproteinurias dropped gradually. The -results suggested that amikacin of routine, dose had no damage to glomerulus, but had a slight damage to proximal and distal tubuli renales contorti,
