血、尿THP放射免疫分析的医学诊断

摘　　要：用ＴＨＰＲＩＡ药盒，对１０１名正常人的血、尿ＴＨＰ进行检测：血ＴＨＰ为１５９．８９±５ｌ．５７ｎｇ／ｍｌ，尿ＴＨＰ为２８．８４±１４．９９ｍｇ／２４ｈ，与国内外学者报道相近。对２９８例８种肾脏病患者血、尿ＴＨＰ值进行了检测，尿ＴＨＰ值均比正常人有明显下降。经统计学处理，慢性肾衰、间质性肾炎、慢性肾炎、狼疮性肾炎、糖尿病性肾病、尿路感染等７种疾患均有非常显著差异；而血中ＴＨＰ值只有前４种有非常显著差异，其余无差异，提示尿样检测优于血样检测。在对２７９例肾病患者检测血、尿ＴＨＰ值的同时，还检测了血、尿β＿２－ＭＧ、尿Ａｌｂ、血液尿素氮（ＢＵＮ）和血Ｃｒ。血、尿ＴＨＰ值对各种肾病的总异常率低于尿Ａｌｂ和β＿２－ＭＧ，但高于ＢＵＮ和血Ｃｒ。The Tamm-Horsfall protein(THP)in bloob and urine of 10 1 healthy subjects and29 8 cases ofpatients with 8 different kinds of diseases are de tected by RIA kit.The average values of THP inheaIthy subjets' bIood and urine are l 59.89±51.57 ng/ml and 28.84±14.99 mg/24 h respective-ly,It is the same as detected by other authors. The resuIts show that the level of urine THP of Pa-tients ,including chronic renal failure,interstitiaI nephritis,chronic nephritis,lupus nephritis,diabet-ic nephritis and urinary infections,are much lower than that of heaIthy subjects(P<0.001),but thelevel of blood THP,are only lower in 4 kinds diseases,e.g.chronic renal failure,interstitial nephri-tis,chronic nephritis and lupus nephritls.In addition,theβ_2-MG in blood and urine,albumin in urineand BUN,Cr in blood in 27 8 cases are detected.It is found that the total abnormal rate of the bloodand urine THP is quite lower than that of urine albumin and β_2-MG,but higher than blood BUN andCr.
