美托洛尔对慢性心力衰竭患者β受体密度影响的临床研究

摘　　要：【目的】探讨美托洛尔对慢性心力衰竭患者心功能的影响及其与β受体密度的关系。【方法】68例 慢性心力衰竭患者,其中32例(对照组)予常规治疗,36例(美托洛尔治疗组)在常规治疗基础上加美托洛尔 治疗。采用3H CGP12177放射配基法检测外周血淋巴细胞β受体以及心脏超声检测左室射血分数(LVEF) 的变化。【结果】美托洛尔治疗组改善心功能的有效率(81.8%)与对照组有效率(59.4%)比较差异有显著性 (P<0.05);美托洛尔组治疗2个月后β受体密度和LVEF与治疗前比较差异有显著性(P<0.05)。非美 托洛尔组治疗前后β受体密度和LVEF的改变差异无显著性(P>0.05)。【结论】美托洛尔能显著改善慢性 心力衰竭患者心功能;β受体密度的上调是预示美托洛尔治疗有效的一个敏感指标。ObjectiveTo study β-adrenergic receptor density of lymphocyte and left ventricular ejection fraction(LVEF) in chronic heart failure patients with metoprolol.In sixty-eight cases with chronic heart failure, 32 cases were treated with traditional methods(control group) and 36 cases treated with traditional methods and metoprolol(treatment group). The binding of β-adrenergic receptor was measured in using 3H-CGP12177 as ligand. LVEF was measured by echocardiography.Compared with control group, the cardiac function in chronic heart failure patients treated with metoprolol was significantly improved. The β-adrenergic receptor density and LVEF were increased at the second month in treatment group, but there was no change in control group.[Conclusion]Treatment of chronic heart failure with metoprolol can obviously improve cardiac function. The β-adrenergic receptor density of lymphocyte up-regulation is a sensitive index of metoprolol effectiveness.
