益肾泻浊方治疗慢性肾衰的临床与实验研究

摘　　要：探讨中药益肾泻浊方(主药:黄芪、太子参、生地、淫羊藿、黄精、红花、炙水蛭、制大黄 )治疗慢性肾功能衰竭(CRF)的疗效与机制。32例患者以该方治疗 1a,并与 28例包醛氧化淀粉治疗的相同病例比较。观察两组患者治疗前、后证候及BUN、Scr的变化,用 5 /6肾切除的方法诱导大鼠肾小球硬化模型并分别给予益肾泻浊方及依那普利治疗 6周,观察大鼠BUN、Scr、IL-6、TNF和残余肾组织的病理改变。结果:中药组患者治疗后临床证候明显改善,BUN、Scr明显下降。西药组治疗前后对比,各项指标均无明显差异。动物实验显示,益肾泻浊方能明显改善模型鼠肾功能,肾脏组织学损伤也相应减轻,且疗效优于依那普利组。提示:益肾泻浊方能有效改善患者临床证候及肾功能,延缓肾小球硬化模型鼠的病变进程。This paper studied the clinical efficacy of “Yishen Xiezhuo Decoction” in the treatment of chronic renal failure(CRF) and its mechanism. Thirty-two subjects were treated with this Decoction for one year and another 28 subjects were treated with Coated Aldehyde Oxystarch; the clinical symptoms, BUN and Scr were observed. Rat model of glomerulosclerosis by five-sixths nephrectomy were treated respectively by this Decoction and Enalapril for six weeks; the BUN, Scr, IL-6, TNF and pathological changes of residual kidney were detected. Results: In CRF patients, the clinical symptoms improved, BUN and Scr fell obviously after treatment by this Decoction; in experimental study, this Decoction could improve rats' kidney function and relieve kidney impairment, with better effect than Enalapril. It is revealed that this Decoction can improve the clinical symptoms and kidney function, and delay the progression of glomerulosclerosis.
