经尿道气化加电切术治疗重度前列腺增生症

摘　　要：目的探讨重度前列腺增生症(BPH)的有效治疗方法。方法应用铲状电极经尿道气化术联合电切术治疗128例重度前列腺增生症,其中Ⅲ度增生107例,Ⅳ度增生21例。结果手术操作平均时间89min,均未发生电切综合征(TURS);切除前列腺组织重量平均41g,无手术死亡;随访3～12个月,IPSS评分从术前平均306分下降至术后平均84分。结论经尿道前列腺气化术(TUEVP)加经尿道前列腺电切术(TURP)联合治疗BPH兼有两者优点,是治疗重度BPH的有效方法。Objective To study the effect of the combined use of transurethral electrovaporization of the prostate(TUEVP) and transuerthral resection of the prostate(TURP) in the treatment of large volume benign prostatic hyperplasia(BPH).Methods Shovel-shaped elec-trode transurethral vaporization combined with electric prostatectomy therapy was performed in 128 cases of large volume BPH.Amony them,grade Ⅲ in 107 cases,and grade Ⅳ in 21 cases.Results Average operation time was 89 minutes.No one suffered from transurethra prostatomy syndrom.Average weight of dissected prostatic tissue was 41g and no death occurred.IPSS scoring decreased from 30.6 to 8.4 after operation during 3～12 months follow-up in 128 cases.Conclusion This procedure has the advantages of both TUEVP and TURP and is safe and effective with fewer complications.It is believed that combined use of TUEVP and TURP is an ideal treatment for large volume BPH.
