摘 要:目的了解生物自动反馈式血容量监测对评估血液透析患者干体质量的作用。方法对26例规侓性血液透析患者,先后按标准透析和生物自动反馈式血容量监测两种方式进行血透治疗,观察两种透析方法透析前后血压、体质量、超滤量、相对血容量的变化和临床合并证及护理干预等指标。结果两组透析前的血压比较没有统计学意义,血容量监测组的干体质量、脱水量、透后血压、急性并发症及护理干预次数与标准血液透析组相比均有显著性差异。生物自动反馈式血容量监测能有效地评估干体质量;有效地减少透析中低血压及肌肉痉挛等低血容量相关不良反应;在下调干体质量后高血压得到改善。结论在透析过程中,根据患者临床表现结合血容量监测有助于确定合适的超滤量、调整干体质量、防治透析过程中低血压和指导治疗顽固性高血压。Objective To evaluate the effect of automatic biofeedback regulation of blood volume monitor in estimating dry weight of hemodialysis patients. Methods 26 chronic hemodynamically stable dialysis patients received standard hemodialysis(SHD) and automatic biofeedback regulation of blood volume monitor(BVM) hemodialysis. Several indexes were observed before and after hemodialysis, such as blood pressure, weight, ultrafiltration volume, posthemodialysis blood pressure, acute complications, nursing treatment and so on. Results Significant differences were observed between BVM and SHD groups in dry weight, ultrafiltration volume, post-hemodialysis blood pressure, acute complications and nursing intervention, while no significant difference in pre-hemodialysis blood pressure. Dry weight was effectively assessed, hypovolemic hypotension and muscle convulsion were reduced, and good blood pressure control was achieved by automatic biofeedback regulation of blood volume monitor. Conclusion It is helpful to determine an appropriate ultrailtration volume, adjust dry weight, prevent from hypotension episodes and treat drug-resistant hypertension in patients with maintenance hemodialysis according to BVM and clinical manifestations during hemodialysis.
