多支动脉桥在冠状动脉搭桥术中的应用

摘　　要：目的　总结近来所做的含有多支动脉桥的冠状动脉搭桥术 (CABG)。方法　在 5例CABG术中选择右侧内乳动脉或桡动脉代替大隐静脉做为移植材料 ,共搭桥 15根 ,其中动脉桥 13根 ,静脉桥 2根。手术注意微创操纵 ,围术期静脉使用钙通道阻滞剂。结果　术中观察动脉桥均通畅 ,无围术期心梗 ,无手术并发症或死亡。术后近期随访心绞痛症状均消失 ,活动量明显增加。结论　选择右侧内乳动脉或桡动脉代替大隐静脉做为移植材料 ,行含有多支动脉桥的CABG术 ,并发症少 ,近期效果良好。Objective To summarize and report the cor on ary artery bypass grafting (CABG) utilizing the multiple arterial conduits in re cent period.Methods Right internal mammary arteries and r adial arteries were taken as grafts, instead of saphenous vein, in 5 CABGs. Fift een grafts were performed, including 13 arterial conduits and 2 vein grafts. Min imily invasive surgery was conducted, and calcium channel blockers were used per ioperatively.Results All arterial conduits were patent. T here were no perioperative myocardial infarct, no operative complications and de ath. Postoperatively, there were no angina any more in all cases, and physical a ctivities increased in recent follow-up.Conclusion Choosi ng right internal mammary arteries and radial arteries as grafts instead of saph enous vein, CABG is performed containing multiple arterial conduits with less co mplications, and with satisfactory recent outcomes.
