小切口胆囊切除术2612例临床分析

摘　　要：目的　探讨小切口胆囊切除术 (MC)的手术方法。论证MC临床应用的可行性。方法　通过对我院 1993年 3月至 2 0 0 3年 2月 ,小切口胆囊切除术 2 612例的临床分析 ,介绍美蓝蓝染法在MC手术中的应用。结果　全组小切口手术成功 2 5 3 0例 (96.86%) ,延长切口 82例 (3 .14 %) ,严重并发症有 :胆道损伤 5例 (0 .19%) ,十二指肠及横结肠损伤 3例 (0 .11%) ,术中大出血 4例 (0 .15 %) ,漏胆 18例 (0 .69%) ,死亡 1例。结论　只要严格掌握手术适应证 ,在传统胆囊切除术熟练的基础上施行MC手术是安全可行的。Objective To report the methods of operation for minicholecystectomy and investigate the clinical practice value of minicholecystectomy.Methods To recommend the employ of Methylene blue staining in minicholecystectomy,wo have studied 2800 cases of MC from March 1990 to February 2003 in our hospital.Results The 2530 cases(96.86%) were cured with minicholecystectomy,82 cases (3.14%) incision lengthened.Serious of complications 5 cases (0.19%) of bile duct injury,3 cases (0.11%) of duodenum and Transverse colon injury,4 cases (0.15%) of massive haemorrhage,18 cases (0.69%) of leak bile,1 case of deadth.Conclusion As long as the operation is applied strictly according to what we emphasized,to skillfully operate the convertional cholecystectomy in basis could be safely performed with minicholecystectomy.
