荧光定量PCR法检测中国籍海员血清HBV DNA

摘　　要：〔目的〕　了解远洋海员HBVDNA的实际感染情况 ,为国境口岸传染病监测策略提供基础数据。〔方法〕　选广州某口岸 2 0 0 3年 3月至 12月中国籍远洋海员体检血清标本 192例 ,用ELISA法检测 192份血清乙肝五项免疫学指标 ,以HBSAG阳性 12 1例为实验组 ,5 1例HBSAG阴性为对照组。用实时荧光定量PCR法同时检测HBVDNA病毒含量。比较两组HBVDNA检出差异。〔结果〕　 192名远洋海员HBVDNA检出 78例 (40 % )。 5项血清免疫学指标和HBVDNA全部阴性 18例。实验组HBVDNA阳性 6 4例 (5 0 .0 2 % ) ,对照组HBVDNA阳性 14例 (2 7.4 5 % ) ,两组差异有显著意义 (χ2 <0 .0 1)。 14例HBSAG阴性HBVDNA阳性血清中 ,现行法规规定的三项传染性指标均为阴性 ,核心抗体均为阳性。〔结论〕　三项传染性指标阴性的人群仍有传播HBV的危险 ,建议有关部门修订远洋海员等特殊人群乙肝防治政策 ,直接检测HBVDNA。睴bjective〕 The objective of this article was to realize the applicability of 3 epidemical indexes of Hepatitis B virus prescribed by active laws and regulation,thus provide data for the establishment and revision of strategies on the control and prevention of HBV in special crowds.〔Methods〕 192 serum samples of Chinese sailors obtained in a physical examination in a Guangzhou port were tested using ELISA method to discover the 5 immune indexes.and using fluorescence quantitative PCR method at the same time to study the amount of HBV DNA.Contrast experiment in which HBSAG positive serum samples were detected as test group and HBSAG negative serum as control,was performed to find out the differences of detected HBV DNA positive ratio.〔Results〕 78 samples were proved to be HBV DNA positive(40%) from total 192 serum samples of the sailors.There were 18 samples with negative results on the 5 serum immune indexex confirmed to be HBV DNA negative.In the contrast with negative results on the 5 serum immune indexes confirmed to be HBV DNA negative.In the contrast experiment,64 samples (50.02%) were proved to be HBV DNA positive in the test group,while there were only 14 samples(27.45%)positive in the control guoup,which meant a significant difference(X^2<0.01)between the 2 groups.We found out that the 14 HBV DNA positive serum samples from the HBSAG negative ones,whose 3 epidemical indexes prescribed by active laws and regulations were all proved th be negative,had HBCAB positive results.〔Conclusion〕 There still exist rist risks on HBV infection from those crowd whose 3 epidemical indexes have been proved to be negative.Thus in this article it is strongly recommended that the involved government departments and organizations should extablish and revise the certain policy on special crowds such as sailors for control and prevention of Hepatitis B virus,and perform HBV detection directly using HBV DNA index.
