POSSUM评分系统预测ICU术后病人死亡率的护理学意义

摘　　要：目的　探讨POSSUM评分系统对ICU术后病人死亡率预测的护理学意义。方法　分析 2 5 2例进入ICU治疗的外科术后病人的死亡率情况 ,用POSSUM评分系统进行评分 ,并按Copeland死亡率预测公式计算 ,比较实际死亡人数与预测死亡人数。结果　本组死亡 5 4例 ,平均年龄 (5 5± 19)岁 ,ICU住院时间 (2 .2± 1.8)d ,生理学评分 (45 .0± 17.1)分 ,手术侵袭度评分 (2 3.0±8.5 )分 ,存活病例 198例 ,平均年龄 (45± 2 2 )岁 ,ICU平均住院时间 (1.8± 1.7)d ,生理学评分 (2 0 .5±9.7)分 ,手术侵袭度评分 (10 .5± 2 .5 )分 ,差异有显著性 (P <0 .0 5 )。按Copeland公式预测死亡率人数为 75人 ,实际人数为 5 4人 ,差异无显著性 (P >0 .0 5 ) ,Copeland公式计算病人死亡率与实际死亡率相符。结论　POSSUM评分系统对预测ICU术后病人死亡率有重要的护理学意义 ,对于临床制订个体化的护理方案具有指导作用。Objective\ To evaluate the clinic significance of POSSUM scoring system in predicting the death rate for surgical postoperative patients in ICU. Methods 252 postoperative patients in ICU were analyzed with the POSSUM scoring system ,A comparison between the observed and the predicted mortality was made. Results In death group(n=54) and survival group (n=198),the average age of the patients were(55±19)y and (45±22) y,the mean length of stay were (2.2±1.8)d,and (1.8±1.7)d,respectively. In the death group, both the physiological scores and operative severity scores of POSSUM scoring system were significantly higher than the survival group (45.0±17.1) vs (20.5±9.7) in physiological score and (23.0± 8.5 )vs (10.5±2.5) in operative severity score (P<0.05). The death rate is 75 persons predicted by Copeland formula, in fact there are 54 ones, there is no significant difference about that, the death rate calculated by Copeland formula is a match case with the actual death rate. Conclusion POSSUM scoring system may be an important role in predicting the mortality for surgical postoperative patients in ICU. It can guide to make out the individual clinic care.
