急性氟乙酰胺中毒患者心肌酶与尿氟水平动态变化研究

摘　　要：目的:探讨氟乙酰胺中毒患者心肌酶和尿氟含量变化.方法:利用全自动生化仪器及氟离子电极法对急性氟乙酰胺中毒患者从住院第1至10天进行血清心肌酶(肌酸激酶-CK、肌酸激酶同功酶-CK-MB、乳酸脱氢酶-LDH、谷草转氨酶-AST、羟丁酸脱氢酶-HBDH)以及尿液总氟动态测定.结果:中毒患者入院第1天至第8天5种酶测定值均显著高于正常对照(P＜0.01),在第3天和第4天出现酶谱峰值,含量达到对照组的2～30倍不等,其中肌酸激酶、肌酸激酶同工酶水平升高最明显,出现最早、维持时间长;中毒患者尿氟含量明显高于正常对照(P＜0.01),与心肌酶水平的升高度相关(r=0.66、0.65、0.88、0.96、0.49);轻度组、中度组和重度组中毒患者之间心肌酶、尿氟含量均有显著性差异(P＜0.01).结论:氟乙酰胺中毒患者心肌酶水平显著增高,这种增高与中毒程度和病程发展密切相关,提示氟乙酰胺中毒越严重,它的'滞留毒性'越强,对心肌损害越严重.Objective:To discuss the changing of serum myocardial enzyme level and urine fluorin level in the patients with acute fluoroacetamice poisoning (AFAP).Methods:The level of serum myocardial enzyme including CK, CK-MB, AST, LDH and HBDH were measured by Htachi 7080 biochemical automatic analyzer,the urine fluorin by ICE in 25 cases of patients with AFAP from the first to tenth days while admitted in hospital.Results:The levels of serum myocardial enzyme in patients with AFAP were significantly higher than those in controls. The results showed a regularly trendency changes that the peak values of serum enzymes appeared in third and fourth days, it was 2～30 times to the controls; Both the level of serum CK and CK-MB varietied in most at early stage; The value of urine fluorin were different significant among the light, middle and serious groups(P<0.01), and there is a correlative relation between value of urine fluorin and serum myocardial enzyme.Conclusion:The levels of serum myocardial enzyme in the patients with AFAP were higher obviously, which was associated with the extent of poisoning and the course of diseases. It suggested the more significant the enzymes incresed,reflecting the severe in the myocardial damaged in the patients with AFAP. [
