摘  要:目的 为获得正常胎儿心脏结构及其心功能的相关参数。方法 应用彩色多普勒超声心动图检测 2 6 3例胎龄 2 0~ 4 2w正常胎心各腔室径线、各瓣膜口峰值血流速度和主 (肺 )动脉加速时间、射血时间及其比值、每搏输出量、心输出量、射血分数、左 (右 )室短轴缩短率等。结果 ①胎儿时期心脏各腔室大小随着孕周的增长而增大 ,右室 >左室 ,结构上右室占优势 ;②各瓣口峰值血流速度随胎龄增加而升高 ,左室低于右室 ;③二、三尖瓣口血流E峰 <A峰 ,E/A <1 ,心室舒张顺应性低下 ;④肺动脉血流加速时间 (AT)及AT/ET短于主动脉 ,肺动脉压高于主动脉压 ;⑤心脏每搏量及心排血量与胎儿生长发育呈正相关 ;⑥左右室壁厚度无显著差异 ;⑦各参数汉族与壮族无差异。结论 了解正常胎心结构及功能 ,对诊断胎儿心血管畸形具有重要意义。Objective To acquire relevant parameter of normal fetal cardiac structure and function.Methods Two hundred sixty-three normal fetuses with 20~42 weeks of gestation were examined the chamber diameter,every valve peak blood flow velocity,aortic/pulmonary artery acceleration time(AT),ejection time(ET),AT/ET,stroke volume,cardiac output,ejection fraction and left/right ventricular fractional shortening,etc.,by color Doppler echocardiography.Results 1.The measurement value of all cavities ant ventricules of fetal neart were increased correspondingly with the gestational ages,right ventricle>left ventricle,right ventricle occupy a dominant position;2.every valve peak blood flow veloctiy were risen correspondingly with the gestational ages,right ventricle more than left ventricle;3.mitral and tricuspid blood flow E peak velocity<A peak velocity,E/A<1,ventricular diastolic compliance was reduced;4.pulmonary artery blood flow acceleration time(AT) and AT/ET were shorter than that of aortic artery,pulmonary artery pressure higher than aortic artery pressure;5.cardiac stroke volume,cardiac output,growth and development of fetuses were positively;6.There is no significant difference between left ventricular and right ventricular wall thickness;7.There was no difference,between every parameter of the Han nationality and Zhuang.Conclusions To understand normal fetal cardiac structure and function is of great importance for diagnos is of fetal cardiac vascular malformation.
