摘  要:目的探讨胸腰段脊柱损伤伴截瘫患者前路手术及Kaneda器械内固定方法。方法总结20例患者经前路减压及Kaneda器械内固定治疗脊柱损伤伴截瘫的经验。结果20例患者经治疗后,5例不全瘫及3例截瘫患者下肢肌力恢复正常,能下地行走,其余12例患者下肢肌力感觉无明显改善。结论脊髓前路减压可减除脊髓前方压迫,为神经功能的保护或恢复创造了必要条件,而Kaneda器械内固定可早期重建脊柱的稳定性。Objective To study the anterior decompression of spinal cord and internal fixation by Kaneda system in thoraco-lumbar injury with paraplegia. Methods The spinal cord anterior decompression fixation of Kaneda system was performed in 20 patients with thoraco-lumbar injury and paraplegia and summarized retrospectively. Results After operation and rehabilitation, of the 20 patients,5 with incomplete injury and 3 with complete paraplegia were able to walk by recovery of leg myodynamic,the other 12 cases could not walk and stood up due to unsatisfactory myodynamic improvement of lower extremities. Conclusion Spinal cord anterior decompression can relieve the anterior compression of spinal cord,and provide a necessary conditions for protecting and recovering the neurofunction. The internal fixation by Kaneda system is especially useful in reconstruction the stabilization of thoraco-lumbar spine after anterior decompression.
