摘  要:目的:研究沙浴对兔膝关节骨性关节炎(osteoarthritis,OA)膝关节软骨的超微结构的影响,为中老年人膝骨关节炎的沙浴治疗提供依据.方法:青紫兰兔15只,成功建立OA动物模型13只,其中12只随机分成自由活动组(A)、自由活动加沙疗组(B)和健侧关节沙疗对照组(C).在平均气温32℃情况下,将模型兔下半部分掩埋在干燥的40~50℃的海沙中,1次/d 25~30 min/次.观察沙疗10 d和20 d后膝关节软骨组织在电子显微镜下的改变.结果:石膏固定兔后肢造成膝关节炎,其软骨组织、细胞退变明显;而超微结构改变以软骨细胞破碎的比较多见,有的软骨细胞则增生,基质胶原原纤维明显减少.去除石膏固定,让患肢自然恢复,超微结构改变呈细胞增生逐渐活跃,细胞内粗面内质网明显增多;但仍可见细胞固缩及细胞崩解呈碎片,并有钙化灶.去除石膏固定,在其患肢自由活动基础上加做沙疗后,患侧膝关节活动改善,超微结构改变呈细胞增生进一步活跃,细胞内见大量粗面内质网,细胞表面突起较多,基质纤维排列较致密,灶性稀疏.正常关节沙疗后,软骨细胞形态保持正常.结论:沙疗能够改善局部血液循环,促进炎症消退,促进软骨细胞增生和活跃,加快骨关节炎的软骨组织修复,而对正常关节无不利影响.Objective: To study the effects of sand treatmen t on ultrastructure of cartilage in articular genu with osteoarthritis(OA) and c onfirm the effects of sand treatment on senile OA in articular genu. Methods: A model of genu OA had been established successfully by fixing left hind limbs with gypsum in 13 rabbits. 12 ones of 13 animals were divided r andomly into free movement group(A) and free movement added sand treatment group (B), and their right hind limbs were used as the normal control(C). Another anim al was used as a specimen. When ambient temperature remained mean 32℃, the lowe r half of group B were buried with 40~50℃ dry sand for 25~30 min one time eve ry day. After these animals were treated ten and twenty days, the ultrastructure s of cartilage in their articular genua were observed under electron microscope, effectively. Results: The cartilage tissue and cell catapla sia and ultrastructure changes were observed in these animals' left articular ge nu. Many broken cartilage cells could be observed and matrix collagenous fibril decreases obviously, but some cartilage cells proliferated. After the fixed gyps um were removed and the left hind limbs of the animals could move freely, the pr oliferation of cartilage cells became active gradually and the rough endoplasmic reticula increased remarkably. The cell pycnosis, broken cells and even calcify cells could be found. Compared with group A, the movement of group B treated by sand treatment improved more significantly, and the changes of ultrastructure o f cartilage included more active cell proliferation, more rough endoplasmic reti cula, more cellular surface projections and more matrix fibers. After the group C(normal articular genu) was treated by sand treatment, the shape of cartilage c ells remained normal. Conclusion: Sand treatment could impro ve local blood circulation, relieve inflammation, and promote proliferation of c artilage cell and recovery of cartilage tissues damaged by OA.
