老年慢性阻塞性肺疾病伴呼吸衰竭早期应用无创双正压通气的监测及护理

摘　　要：目的 :观察老年慢性肺阻塞性疾病 (COPD)伴呼吸衰竭早期应用BiPAP呼吸机治疗的效果。方法 :6 2例患者行常规抗感染及祛痰等治疗外 ,加用BiPAP呼吸机通气 ,根据患者的病情选择呼吸机工作模式和参数 ,观察通气前后血气分析结果。结果 :6 2例中 ,5 9例临床症状明显缓解 ,PaCO2 、心率、呼吸频率明显降低 ,PaO2 、SaO2 显著增高。 3例因气道分泌物过多 ,改为有创通气。结论 :早期应用Bi PAP呼吸机通气 ,疗效好 ,病死率低 。Objective:To observe the effect with the BiPAP to treat the patients with chronic obstructive pulmonary disease complicated with respiratory failure at early stage.Method:Sixty -two patients were performed the treatment,include common anti-infection and expectoration,in addition to the BiPAP respiratory apparatus, choose the respiratory apparatus work model and data according to the patients' condition,catch blood gas analysis before that after ventilation.Result:Among the 62 patients,59 pafients' clinical symptoms alleriated obviously, PaCO 2,HR and respiration activity obviously decreased,PaO 2,SaO 2 significantly increased,3 patients were changed to the tranmatic ventilation because of the increasing of the respiratory tract secretion.Conclusion:At the early stage,to apply the BiPAP respiratory apparatus ventilation can gain the good effect , lowed mortalily and shorten hospitalig tion time.
