摘 要:【目的】探讨胰岛素样生长因子Ⅰ (IGF Ⅰ )和胰岛素样生长因子结合蛋白 1(IGFBP 1)与胎儿生长受限 (FGR)的关系。【方法】采用免疫放射法测定 30例胎儿生长受限孕妇 (FGR组 )和 10 8例正常孕妇 (对照组 )的血清及新生儿脐血IGF Ⅰ、IGFBP 1水平 ,并对其结果进行相关性分析。【结果】①FGR孕妇血清及新生儿脐血IGF Ⅰ水平低于正常孕妇 (p <0 .0 5 )、IGFBP 1水平高于正常孕妇 (P <0 .0 5 )。②两组孕妇血清IGF Ⅰ、IGFBP 1水平明显高于新生儿脐血IGF Ⅰ、IGFBP 1水平 (P <0 .0 1)。③两组孕妇血清IGF Ⅰ水平与新生儿脐血IGF Ⅰ水平无关 (r =0 .2 6 2 ,P >0 .0 5 )。④两组孕妇血清及新生儿脐血IGF Ⅰ水平与IGFBP 1水平呈负相关 (r =- 0 .386 ,r =- 0 .32 7,P <0 .0 5 )。⑤新生儿脐血IGF Ⅰ水平与新生儿出生体重呈正相关 (r =0 .5 75 ,P <0 .0 1) ,新生儿脐血IGFBP 1水平与新生儿出生体重呈负相关 (r =- 0 .4 18,P <0 .0 5 )。【结论】①胎儿血循环中低水平的IGF Ⅰ及高水平的IGFBP 1可能是导致FGR的重要病因之一。②两组孕妇血清IGF Ⅰ与新生儿脐血IGF Ⅰ的分泌相对独立 ,IGFTo explore the relationship of insulin-like growth factor-Ⅰ (IGF-Ⅰ) and insulin-like growth factor-binding protein-1 (IGFBP-1) to fetal growth retardation (FGR).Immunoradiometric analysis (IRMA) was used to estimate the serum and cord blood levels of IGF-Ⅰ and IGFBP-1 in 30 fetuses with FGR (FGR group) and 108 normal pregnant women (Control group). Correlation analysis of the results was made.①The serum and neonatal cord blood levels of IGF-Ⅰ in pregnant women of FGR group were significantly lower than those in control group (P<0.05), whereas the IGFBP-1 levels in the former were obviously higher than those in the latter(P<0.05). ② The serum levels of IGF-Ⅰ and IGFBP-1 in pregnant women of two groups were distinctly higher than those in neonatal cord blood (P<0.01). ③ The serum IGF-Ⅰ levels in pregnant women of two groups were not correlated with those in neonatal cord blood (r=0.262, P>0.05). ④ In pregnant women and neonates of two groups , their serum and cord blood IGF-Ⅰ levels were negatively correlative to IGFBP-1 levels (r= -0.386,r=(-0.327),P<0.05). ⑤ The IGF-Ⅰ levels in neonatal cord blood were positively correlative with their birth weights (r=0.575, P<0.01) ; while IGFBP-1 levels in neonatal cord blood were negatively correlated with their birth weight (r=-0.418, P<0.05).[Conclusion]①The low IGF-Ⅰ and high IGFBP-1 levels in neonatal cord blood are possibly one of the important causes leading to FGR. ② Secretions of serum IGF-Ⅰ in pregnant women and cord blood IGF-Ⅰ in neonates of two groups are relatively independent, and IGF-Ⅰ can not pass though the placenta barrier.
