摘 要:目的 :研究急性高浓度氯气染毒后血浆纤维结合蛋白 (FN)含量的变化及其在肺损伤中的作用。方法 :35只家兔随机分成整体组和离体肺组 ,两组再分对照组和氯气染毒组。整体组行气管插管、开胸行肺动脉主干插管 ;离体肺组动物放血后取出心肺进行灌流、通气。染毒组动物吸入 5 0 0 0ppm的氯气 (2 0min)后 ,定时检测血浆 /灌流液内FN(pFN)含量、肺动脉 /灌流压力、呼吸功能指标 ,监测离体肺重量并计算肺水指标。结果 :吸入氯气致离体肺重量显著增加 ;肺水指标均显著升高 ,血浆 /灌流液内FN的含量显著下降。整体动物的呼吸频率加速 ,潮气量、血氧分压显著下降。结论 :氯气染毒引起了严重肺损伤、肺水肿 ,血浆 /灌流液内FN含量显著下降 ,FN含量下降程度与肺损伤程度相关。Objective:To investigate the mechanisms and significance of plasma fibronectin(pFN)concentration change in intact and isolated perfused rabbit lungs(IPLs)following acute exposure to high concentration of chlorine gas by inhalation.Methods:Thirty-five rabbits were randomly divided into 4 groups:intact control group(n=8),intact chlorine gas exposure group(n=8),isocated control group(n=9)and isolated chlorine gas exposure group(n=10).Endotracheal intubations and aorto-pulmonary intubations were performed in intact control group and intact chlorine gas exposure group.Perfusion and ventilation were performed in isolated control group and isolated chlorine gas exposure group after the heart and lungs were taken out.In both intact and isolated chlorine gas exposure groups,concentration of pFN,Lung weight of IPS(△WT),pressure of pulmonary artery(Pa)and venous pressure(Pv),airway pressure and tidal volume were all periodically measured after the rabbits were exposed to high concentration(5000ppm)chlorine gas for 20 minutes.Resuits:The concentration of pFN decreased slightly with time in intact control group but did not change in isolated control group,however,it decreased significantly after the rats were exposed to chlorine gas in both intact and isolated chlorine gas exposure groups.Following the exposure to chlorine gas ,the respiratory rate in intact rabbits was accelerated immediately and vidal volume decreased.Conclusion:Inhalation of chlorine gas gives rise to serious lung injuries and edema and significant decrease of pFN.The degree of the decrease of pFN concentration is directly proportional to the degree of lung injuries.
