致病性鳞翅目刺蛾幼虫形态及皮炎患者临床症状研究

摘　　要：目的:探讨致病性鳞翅目刺蛾幼虫形态及所致皮炎患者临床症状,为采取早期控制措 施提供科学依据。方法:用老熟茶毛虫触刺豚鼠的腹面皮肤、自愿者上肢及自然染毒者,动态观察茶 毛虫皮炎的病程,对豚鼠皮炎病灶作常规病理学观察,比较豚鼠、自愿者染毒前、后血象变化。结果: 自愿者皮肤被幼虫触刺后,表现为红、肿、痛、热及烧灼、麻木感,继而呈现丘疹、斑丘疹、斑块、风团块, 部分病例出现周身不适(76.64%)、烦躁(32.71%)和腋下淋巴结肿大(16.82%)。豚鼠皮疹病理切片 见表皮水疱、脓疱改变。染毒后白细胞、嗜酸性粒细胞高于染毒前。结论:鳞翅目刺蛾幼虫具有致病 性,致病机理为机械刺伤、生物毒性和变态反应。Objective: To study the pathogenicity of lepidoptera eucleidae caterpillar and the clinic symptom of the dermatitis so as to provide scientific basis for make control measure early. Methods: Case-control study was used. The guineapigs abdomen and the skin of control group were being touched directly by mature tea euproctis pseudoconspersa strand on their upper limb or nature catch poison. The pathogenic course was observed continuously in human with dermatitis which was caused by tea euproctis pseudoconspersa strand. Pathological local lesion of guineapigs was cut off in time, and observed with routine histology. Furthermore, compare WBC and addicted acid granule cell change pre and post of poisoning. Results: Following symptoms with post poisoning included reddish, turgescence, pain, heat, burning pain and numbness. Accompanied by the above symptoms, papule , macule , maculepapule , urtica and plaque also occurred, even in part of controls onset of the felling unwell of the whole body (76.64%), dysphoria(32.71%), and axillary lymphadenovarix (16.82%). There was epidermis bulla , vesicle and pustule on routine cut section of lesion of guineapigs. The WBC and addicted acid granule cell were higher in pre-poisoning than in post-poisoning. Conclusions: It was shown that the eucleidae caterpillar did have pathogenicity. Pathogenic mechanism in human dermatitis was machanical injury, biology poisonous reaction and allergy.
