女性混合性尿失禁的综合治疗

摘　　要：目的　评价女性混合性尿失禁综合治疗的疗效。方法　从 2 0 0 3年 7月至 2 0 0 4年 5月间 ,本院女性尿失禁特需门诊收治确诊为混合性尿失禁患者 2 3例 ,年龄 4 7～ 80岁 ,平均 6 6岁 ,采用手术、留置导尿 72h、行为心理疗法及药物使用的综合治疗 ,对治疗后的效果进行定期随访。结果　全部患者在完成手术后 ,72h去除导尿管能自行排尿 ,96h后导尿测残余尿 <10 0ml,急迫性尿失禁症状控制满意后出院 ,2 3例中 2 2例持续随访 ,随访时间 2～ 9个月 ,平均 4 .5个月 ,压力性尿失禁无复发 ,急迫性尿失禁症状 18例明显改善 ,4例较以往有较大改善。无其他并发症发生 ,主观满意 ,无尿潴留、尿路感染和膀胱功能障碍。结论　女性混合性尿失禁的综合治疗以手术为主 ,辅以其他方法 ,效果肯定 ,但长期效果有待进一步随访。Objective To evaluate the effect of syndrome thraphy in treatment of female mixed urinary incontinence.Methods From July 2003 to May 2004,23 patients with mixed urinary incontinence,with age ranging from 47 to 80,were treated with operation,in-dwelling catheter for 72 hours,drug therapy and behaving or psychology therapy respectively.Patients were followed up and treatment effect was evaluated.Results All of the patients were able to micturate spontaneously at 72 hours after operation.The volume of urine remained was not over 100 ml.According to patient's self-assessment,all the patients with urge urinary continence were discharged when they felt being cured.22 of 23 patients were followed up in these cases,all the stress urinary incontinence patients felt being cured after followed up for 9 months.18 urge urinary incontinence patients felt obviously alleviative and 4 case improved.There was no urine retentiom,infection or bladder dysfunction.Conclusion Syndrome therapy including operation and others is a very effective method in treating mixed urinary incontinence,but its long-standing result must be evaluated in the future.
