血管内皮生长因子及其受体在银屑病发病中的作用及意义

摘　　要：目的研究血管内皮生长因子(VEGF)及其受体在银屑病皮损中的表达。方法采用原位杂交和免疫组化SABC法,检测42例寻常性银屑病患者皮损和15例正常人皮肤组织VEGFmRNA和其蛋白、VEGF受体2(KDR蛋白)、微血管密度(MVD),分析其表达情况。采用双抗体夹心酶联免疫吸附法检测患者和正常人血清中VEGF水平。结果①寻常性银屑病患者皮损处的VEGFmRNA及其蛋白、KDR和MVD表达明显强于正常人对照组(P<0.001);皮损处与非皮损组相比,VEGFmRNA和其蛋白的表达差异无统计学意义,但KDR、MVD的表达差异有统计学意义;进行期与静止期比较四者差异均有统计学意义。②高密度组(MVD≥17.88)与低密度组(MVD<17.88)的VEGFmRNA及其蛋白表达差异无统计学意义,但KDR表达差异有统计学意义(P<0.01),且PASI评分差异也有统计学意义(P<0.001)。③患者血清中VEGF水平明显高于正常人对照组。结论①VEGF及其受体在银屑病新生血管形成中起重要作用。②MVD值可反映寻常性银屑病的严重程度,可作为判断病情和预后的指标。Objectives To investigate the relationship between angiogenesis and vascular endothelial growth factor (VEGF) and its receptor in patients with psoriasis. Methods The expression of VEGF and its receptor KDR, and micro-vascular density (MVD) were measured in 42 patients with psoriasis vulgaris and 15 normal controls by in situ hybridization and immunohistochemistry technique, respectively. The concentration of VEGF in sera was measured by double antibody sandwich enzyme-linked immunosorbent assay (ELISA) in patients with psoriasis vulgaris and normal controls. Results ①The expression of VEGF mRNA and its protein, KDR, and MVD in psoriatic lesions were significantly higher than those in the normal controls (P < 0.01). Between psoriatic lesional and non-lesional skins, there were no significant difference in the expression of VEGF mRNA and its protein, while the difference was significant in the expression of KDR and MVD (P < 0.05). There were significant differences in the expression of VEGF mRNA , its protein, KDR and MVD between the active and the resting stages of psoriasis vulgaris (P < 0.05 ). ②There were no significant differences in the expression of VEGF mRNA and its protein between the high MVD (≥17.88) and the low MVD (< 17.88) groups, but the expression of KDR in the high MVD group was higher than that in the low MVD group (P < 0.01), and the difference of PASI scores was significant in the two groups (P < 0.001). ③Serum concentration of VEGF was significantly higher in psoriasis patients than that in the normal controls (P < 0.001). Conclusions ①VEGF and its receptor might play an important role in the angiogenesis in psoriasis. ②MVD is related to the seriousness of the disease, and might be a useful index for judging the status of the disease and prognosis of the patients with psoriasis vulgaris.
