鼻咽癌中医辨证分型与预后相关性基因活性表达关系的初步研究

摘　　要：目的 探讨鼻咽癌辨证分型与其原发灶癌组织中预后相关性基因活性表达的关系及其对患者预后的意义。方法对60例初诊鼻咽癌患者进行辨证分型,并检测其鼻咽活检标本nm23-H1、VEGF(voscular endoth-elial growth factor,血管内皮生长因子)基因表达活性,并于治疗后随访5年,分析其预后相关性。结果 辨证分型结果,气阴两虚型患者30例(50.0％),气血凝结型18例(30.0％),火毒困结型12例(20.0％)。气阴两虚型组鼻咽活检标本的nm23基因表达阳性率50.0％,VEGF基因表达阳性率70.0％,分别明显低于或高于其他两组(P<0.05),且其5年存活率低,累计复发率高。结论 初诊鼻咽癌气阴两虚证患者nm23-H1活性表达下调,vEGF活性表达上调,预后不良。Objective To investigate the correlation of patterns in the terms of TCM with the expressive activities of prognosis-related genes in the biopsied primary cancerous tissues among the untreated cases with nasopharyngeal carcinoma (NPC). Methods Sixty cases with NPC were included in this study,with the diagnosis being made on the basis of histopathology to the biopsied primary cancerous tissues in the nasopharynx. The patterns of them were differentiated based on the clinical data collected on the first consulting and the expressive activities of prognosis-related genes,i. e. nm23-H1 and VEGF, were determined on the samples biopsied from the primary cancerous tissues. Then,all the cases were followed up for more than 5 years, or till the death of any case was confirmed, after treatment. Followed were an analysis among these patients for the correlation of patterns with the prognosis of them by the end of following-up period. Results In this group of cases, 30(50. 0%) were diagnosed with the pattern of dual deficiency of Qi and Yin,18 were with that of Qi and blood congealing,and 12 with one of fire and toxin bounding. The patients with a pattern of dual deficiency of Qi and Yin showed significantly lower rate of nm23-H1 expression(50. 0%) and higher rate of VEGF expression(74%) in the primary cancerous tissues,than that among the cases of other two kinds ot pattern respectively(P<0. 05). Conclusion The untreated NPC patients with the pattern of dual deficiency of Qi and Yin may show a poor prognosis on the basis of down-regulated expressive activity of nm23-H1 and up-regulated activity of VEGF expression.
