摘  要:目的:探讨Th细胞介导的免疫应答在自身免疫性扩张型心肌病(DCM)发生中的作用趋势。方法:应用 线粒体ADP/ATP载体肽(免疫应答组)和不含肽的免疫液(对照组)免疫Balb/c小鼠,试验期内(6个月)光镜和电 镜观察心肌组织病理学变化,ELISA法检测其血清抗ADP/ATP载体自身抗体及IgG亚类水平,三色荧光标记的流 式细胞术测其脾细胞中Th细胞亚群分布。结果:免疫应答组小鼠自身抗体均为阳性,且在免疫过程中其抗体水平、 血清IgG1及IgG2a亚类、Th1/Th2细胞亚群含量均持续高于对照组(P均<0.01)。免疫的第1个月末,免疫应答 组小鼠心脏形态及病理学变化类似心肌炎,IgG2a亚类与Th1细胞亚群在体内占优势;第6个月末,心脏改变类似 扩张型心肌病,且以IgG1与Th2亚群为主,对照组均正常。结论:线粒体ADP/ATP载体肽可诱导小鼠血清IgG2a 向IgG1优势的转化,此转化与Th1、Th2免疫应答分别在自身免疫性DCM发生的早期和后期阶段起优势作用有关。Aim:To explore the contribution of Th immune responses in the development of autoimmune dilated cardiomyopathy(DCM). Methods: Cardiac morphological and histological changes were observed in mice with DCM induced by the mitochondria ADP/ATP carrier peptides (DCM-group) and the sham-immunized controls under light and electron microscopes. At the same time, serum autoantibody against ADP/ATP carrier and IgG subclasses were measured using the method of ELISA.Splenic Th cell subsets were detected using the three-color flow cytometry.Results: In DCM-group, autoantibody against ADP/ATP carrier was positive in all mice, and the levels of this autoantibody, serum IgG1 and IgG2a subclasses and Th1/Th2 cell subsets remained higher than control group during the course of immunization (P<0.01). Furthermore, the predominance of serum IgG2a and splenic Th1 cell subset existed till the end of the lst month, whereas serum IgG1 and splenic Th2 cell subset took predominance at the end of the 6th month of the immunization, companied by the pathological changes from myocarditis to DCM. Such findings were not noted in control group. Conclusion: ADP/ATP carrier peptides could induce the switch of IgG from IgG2a to IgG1 in the development of autoimmune DCM, which is associated with Th1 immune response playing the predominant role at the early-phase of the disease while Th2 immune response,at the late.
