支架引导下十二指肠乳头切开的临床探讨(附22例报告)

摘　　要：目的探讨支架引导下胃大部切除术后的十二指肠乳头、憩室内乳头及副乳头的切开效果。方法选择22例病人,在支架引导下用针形刀行十二指肠乳头切开术。结果支架引导下胃大部切除术后乳头切开14例,憩室内乳头切开4例,副乳头切开4例。其中2例切开后有出血,内镜下止血成功。无术后胰腺炎、穿孔等并发症发生。结论支架引导下用针形刀对困难十二指肠乳头切开是一种安全、有效的切开方法。To assess the result of performing sphincterotomy when either BillrothⅡ gastrectomy or complicated periampullary diverticula and in addition the papilla of santorini. Stent-guided needle knife sphincterotomy had been utilized in 22 patients. They were performed in 14 patients who had undergone BillrothⅡ gastrectomy, 4 with periampullary diverticula and 4 with pancreas divisum.Two patients experienced hemorrhage. No mortality pancreatitis and perforation occurred. [Conclusions] The stent-guided needle knife sphincterotomy approach is a safe and an effective technigue. It has a low complication rate even in those difficult anatomic condition.
