缩窄下腔静脉对猪肝脏血流动力学的影响

摘  要:目的 探讨缩窄下腔静脉(IVC)对肝脏血流动力学的影响。方法 2 0只猪麻醉后,采用超声时差血流表测定肠系膜上动脉、肝门静脉、肝动脉、腹腔动脉、脾动脉、颈总动脉及肾动脉血流。结果 下腔静脉缩窄前的基础期与缩窄时的实验期各血流(ml/min)分别为:肠系膜上动脉74 4 .96±16 8.5 3与5 80 .72±16 2 .18(P <0 .0 5 ) ;肝门静脉870 .2 3±2 18.84与72 8.98±2 4 7.2 5 (P <0 .0 5 ) ;颈总动脉15 8.6 7±4 1.6 2与14 3.35±38.2 8(P <0 .0 5 ) ;肾动脉2 12 .38±74 .15与5 1.89±2 2 .89(P <0 .0 1)。但腹腔动脉2 87.0 2±12 8.5 0与2 74 .5 6±132 .0 7、肝动脉16 5 .4 5±76 .83与15 2 .15±78.73和脾动脉6 4 .0 8±2 5 .79与5 7.13±2 1.78缩窄前后均无显著性差异(P >0 .0 5 )。结论 缩窄猪IVC后肝门静脉、肠系膜上动脉、肾动脉和颈总动脉血流显著减少,而腹腔动脉、肝动脉和脾动脉血流无显著变化。物理的流体力学因素在肝动脉缓冲效应的发生机制中可能起着重要作用。Objective To discuss hepatic hemodynamics during inferior vena cava(IVC) was occluded. Methods Blood flows of superior mesentery artery (SMA),liver portal vein (PV) ,hepatic artery(HA),celiac artery(CA), splenic artery (SA),common carotid artery (CCA) and renal artery(RA) were measured with ultrasonic transit time flowmeter in 20 anesthesia pigs by occluding IVC. Results Basic period before occlouding IVC vs during occluding experimental period blood flows (ml/min) were in the following: SMA 744.96±168.53 vs 580.72±162.18,(P<0.05),PV 870.23±218.84 vs 728.98±247.25,(P<0.05),CCA 158.67±41.62 vs 143.35±38.28,(P<0.05),RA 212.38±74.15 vs 51.89±22.89,(P<0.01),CA 287.02±128.50 vs 274.56±132.07 (P>0.05),HA 165.45±76.83 vs 152.15±78.73 (P>0.05) and SA 64.08±25.79 vs 57.13±21.78 (P>0.05) were not changed significantly. Conclusion The blood flows of SMA,PV,CCA and RA are significantly decreased,but CA,HA and SA are not significantly changed at basic and experimental periods during occluding IVC in pigs.Physics effect of hemodynamics may play an important role in mechanism of hepatic arterial buffer response.
