摘 要:目的:在大鼠心肌梗死模型上移植冠心病患者来源的骨髓间质细胞,并使用超声手段对疗效进行评价。 方法:抽取冠心病患者骨髓,用人骨髓间质细胞专用培养液进行体外培养、扩增、标记。结扎大鼠冠状动脉左前降支制作心肌梗死模型,二周后使用超声筛选出合格的动物模型。然后动物随机分为治疗组(n=14)和对照组(n=13)。细胞移植后给予免疫抑制治疗,并对心脏的收缩、舒张功能和心室重构进行超声评价,取材后进行病理检查。 结果:治疗组和对照组细胞移植前超声检测无显著差异。治疗组较对照组射血分数值和缩短分数值显著改善,局部前壁室壁增厚率明显提高,室腔扩大小于对照组。大鼠心脏舒张功能两组无显著差异。病理检查提示移植的细胞在心肌梗死区内存活。 结论:冠心病患者骨髓间质细胞移植后可以提高心肌梗死后心脏收缩功能。对于小动物的心肌再生研究而言,心脏超声是一种较可靠的心功能评价手段。Objective : To assay the effect of the transplantation of bone marrow stromal cells (BMSCs) from coronary artery disease ( CAD) patients in a rat myocardial infarction model by echocardiography.Methods: Bone marrow were harvested from CAD patiens, and then the BMSCs were isolated, cultured and labeled. Two weeks after LAD ligation, valid rat myocardial infarction models were chosen on echocardiogram. The rats were randomly divided into two groups: cell transplantation (CTX) (n = 14) and control groups(n = 13). Fifty ul cell suspension which contained 2 millions of BMSCs was injected into the infracted myocardium and the same volume serum-free culture medium was injected in the control group. Four weeks after injection, echocardiography was performed to assay the changs of ventricular function and remodeling. The histological study was also performed to identify the implanted cells.Result:There was no significant difference between the two groups on echo cardiogram just before transplantation. Four weeks later, compared with the control group, both systolic heart function and the change of anterior wall thickening of CTX group were significantly improved. The left ventricle cavity was enlarged in both groups,but less enlarged in the CTX group. No significant difference about diastolic function was observed. By histochemical staining, pre-labled cells were identified in the infarction and border zone.Conclusion: Human BMSC can be cultured and expanded for cell transplantation. Transplantation of BMSC into rat myocardial infarction model can promote contractile function. Echocardiography may be a reliable alternative approach to assay rat heart function after cellular cardiomyoplasty.
