摘  要:目的获得滑车神经的MRI断层影像解剖资料,研究滑车神经的解剖走行特点及与神经血管关系.方法应用3D-CISS序列,结合3D-TOF 序列,对40位受验者(80条神经)及1位上斜肌纤维痉挛(SOM)患者进行MR成像分析研究.结果 80条滑车神经在横断位、矢状位、冠状位分别以74条(93%)、66条(82%)和63条(79%)的几率显示.滑车神经在脑池内的起点到中脑正中矢状面的垂直距离范围是3~8 mm,滑车神经在脑池内的最大显示距离范围是1~23 mm.在10条(12.5%)滑车神经起点区域1.2 mm的范围内分别观察到神经血管接触点;在31条(38.8%)滑车神经远侧距离起点平均约3.4 mm处观察到神经血管接触点.在伴有上斜肌纤维痉挛患者的滑车神经起点处观察到神经血管接触点.结论 3D-CISS序列结合3D-TOF序列能清楚显示脑池段滑车神经的走行特点及神经血管关系.Objective To obtain MRI sectional anatomical data of the proximal portion of the cisternal segment of the trochlear nerve,and to assess the anatomical features and vascular relationships of the trochlear nerve.Methods 40 subjects(80 nerves) and one patient with right superior oblique myokymia(SOM)were examined with 3D-CISS and 3D-TOF sequences.Results 3D-CISS MR imaging depicted the proximal cisternal segment of the trochlear nerve in the transverse, sagittal, and coronal planes in 74(93%), 66(82%)and 63(79%)of 80 nerves respectively. The distance between the midline and the point of exit(PE) was 3~8 mm, and the maximum visualized length of the trochlear nerve was 1~23 mm. An neurovascular contact was seen at the root exit zone(REZ)in 10(12.5%)nerves and at a mean distance of 3.4 mm distal to the exit zone(PE)in 31(38.8%)nerves. The patient with SOM had a neurovascular contact at the REZ. Conclusion Use of 3D-CISS and 3D-TOF sequences enables accurate identifying the proximal cisternal segment of the trochlear nerve and its neurovascular relationships.
