尿道镜在无张力阴道悬吊术中的应用

摘　　要：目的:探讨尿道镜在无张力阴道悬吊术(TVT)中的作用,改进TVT的手术技巧.方法:24例女性压力性尿失禁(SUI)者行TVT术,术中采用传统的嘱病人咳嗽观察悬吊之松紧度,同时使用尿道镜判断悬吊之松紧度.结果:随访3～21个月,18例SUI得到满意的控尿效果;6 例主观症状均明显改善,其中4例有一过性排尿困难,经导尿或尿道扩张均于3周内消失,1例患者术后剩余尿90 ml,1例仍有轻微尿失禁.结论:术中辅助尿道镜观察,有利于悬吊之松紧度判断,提高手术效果.Objective: To explore the application of urethroscope in the procedures of tension-free vaginal tape (TVT) for the treatment of female stress urinary incontinence (SUI).Methods: A total of 24 patients underwent TVT placement for the treatment of SUI which was confirmed urodynamically, and urethroscope was applied in the procedures to determine the propriety site of the TVT placement. Results: 18 of 24 patients were completely cured of their SUI. The rest 6 patients were significantly improved, and among them, 4 patients needed an indwelling catheter for a few days or 1～3 times urethra dilatation in 3 weeks,and 1 case had 90 ml residual urine. 1 case improved in symptom,but remained SUI in some degrees. Conclusion: The assistant of urethroscope in the tension-free vaginal tape procedure is both a safe and effective method to cure SUI.
