吻合血管的腓骨头移植重建外踝的解剖学基础

摘　　要：目的:为带血管蒂腓骨头移植重建外踝提供解剖学基础。方法: 在左右各 30 侧成人干燥腓骨标本上,对腓骨头和外踝进行形态学测量。结果:腓骨头与外踝形态比较,最大周径均值:腓骨头为(7.3±0.9)cm,外踝为(6.7±0.6)cm;关节面横宽均值:腓骨头为(1.4±0.2)cm,外踝为(1.6±0.2)cm;关节面纵长均值:腓骨头为(1.5±0.2)cm,外踝为(2.0±0.2)cm;关节面夹角均值:腓骨头为(144±11)°,外踝为(162±10)°;腓骨头或外踝与腓骨干倾斜角均值:腓骨头为(173±7)°,外踝为(171±6)°。结论:腓骨头与外踝形态相似,设计带膝下外血管蒂腓骨头移植修复外踝具有可行性。Objective: To study the anatomic basis for restructuring lateral malleolus with vascularized fibular head. Methods: Figures of fibular head and lateral malleolus were measured on 60 adult dry fibulas. Results: Compared the measurements of fibular heads to those of lateral malleolus, the average of the max perimeter was (7.3±0.9)cm at fibular head, while( 6.7±0.6)cm at lateral malleolus; the average width was(1.4±0.2)cm at articular surface of fibular head, while (1.6 ±0.2)cm at lateral malleolus; the average length was (1.5±0.2)cm at articular surface of fibular head, while( 2.0±0.2)cm at lateral malleolus; the average angle was (144°±11°) between two articular surface of fibular head, while (162°±10°)between two articular surface of lateral malleolus; the average angle was (173°±7°) between fibula and fibular head, while (171°±6°) between fibula and lateral malleolus. Conclusion: The figure of fibular head is similar to the figure of lateral malleolus. Vascularized fibular head may be applied to restructure lateral malleolus.
