摘　　要：目的:研究男性性病患者念珠菌性尿道炎的流行病学和抗真菌药物疗效。方法:对已确诊为非淋菌性尿道炎、尖锐湿疣、淋病、生殖器疱疹、梅毒等有尿道炎症状者,用患者尿道分泌物或尿道拭子作真菌培养,并观察斯皮仁诺、氟康唑治疗念珠菌性尿道炎的临床治愈率和真菌学治愈率。结果:男性病患者念珠菌尿道炎的感染率由2.50%逐年上升至17.64%,NGU患者中检出最多,共193例(90.61%);念珠菌性尿道炎合并Ct和/或Uu感染者为26.42%;以白念珠菌感染导致念珠菌性尿道炎最为多见,共166例(77.93%);斯皮仁诺、氟康唑治疗187例念珠菌性尿道炎的临床治愈率、细菌学治愈率分别为77.71%、95.18%和80.95%、90.48%。结论:男性病患者念珠菌尿道炎感染率呈逐年上升趋势,NGU患者感染率最高。尤以白念珠菌感染最为多见。斯皮仁诺、氟康唑可以作为治疗念珠菌性尿道炎首选药物。Objective: To study epidemiological characters of candidal urethritis in male STD patients and the effect of antifungal agents on fungi. Methods: Fungi in urethral exudate from patients with STD and urethal symptoms were detected through fungal culture, after STDs including non-gonococcal urethritis(NGU), condyloma acuminatum, gonorrhea, genital herpes and syphilis were diagnosed. And curative effect of itraconazole and fluconazole on candidal urethritis were comparatively analyzed. Results: Infection rate of candidal urethritis rised gradually from 2.50%(1989) to 17.64%(1999), and patients with candidal urethritis were mostly found among with NGU patients. Furthermore, 26.42% patients with candidal urethritis complicated by Ureaplasma urelyticum and/or Chlamydia trachomatis were found. Fungal culture showed most patients with candidal urethritis were infected with Candida albicans (77.93%). Clinical cure rate and fungal clearance rate of candidal urethritis treated with itraconazole vs fluconazole were 77.71% and 95.18% vs 80.95% and 90.48%. Conclusion: Infection rate of candidal urethritis in male STD patients has a predisposition to rise gradually ever since 1989 years, and patients with candidal urethritis are mostly found among patients with NGU. Most of patients with candidal urethritis are infected with Candida albicans . Itraconazole and fluconazole are the first choice and high-effective drugs for candidal urethritis.
