摘  要:目的研究肝小血管瘤(SHHE)在螺旋CT多期(动脉期、门脉期和延迟期)增强扫描中的表现,提高诊断水平。方法40例肝小血管瘤患者行平扫及增强扫描,对比剂注射速率为3ml/s,然后开始动脉期、门脉期和延迟期的扫描,观察病灶的强化方式和特征。结果共发现45个病灶,60.00%的SHHE具有典型表现,如边缘明显强化逐渐向中心充填,35.56%在动脉期和门脉期均为高密度,4.44%多期均为低密度,而在第三期扫描中95.56%的SHHE为高密度和等密度。结论螺旋CT多期扫描可以充分显示SHHE各期的增强表现,正确评价其血流变化,提高了病灶的检出率和诊断的准确率。Objective To investigate the features of the enhancement of small hepatic hemangioma(SHHE) at multiphase spiral CT scanning and improve the diagnosis. Methods The pre- and post-contrast CT scanning of the entire liver in 40 patients with SHHE was carried out with a spiral CT scanner. The contrast material was infused at a rate of 3ml/s, followed by sequential arterial, portal and equilibrium phase scans of the liver, the contrast enhancement patterns and characterization of all lesions in each phase was observed. Results 45 lesions were found in 40 patients, 60.00% of SHHE had typical appearance, such as peripheral areas of lesions enhanced likely small ball, progressive opacification from the periphery to center. 35.56% of SHHE were hyper-dense in multiphase and 4.44% of SHHE were hypo-dense in both phase. In the third phase scanning, 95.56% of SHHE were hyper-dense and isodense. Conclusion Multiphase contrast enhanced spiral CT can fully demonstrate the patterns of SHHE, reflect the blood supply of SHHE, and improve the diagnosis.
