摘  要:目的:探讨用尿红细胞激光参数测定来辅助鉴别血尿来源的临床意义。方法:利用UF-100流式尿 沉渣分析仪对79例肾性血尿(经肾活检证实)标本和128例非肾性血尿标本进行尿红细胞各项激光参数的测定,并将 仪器红细胞信息与普通光镜红细胞形态结果进行比较分析。结果:肾小球性血尿和非肾小球性血尿标本的70%红细 胞前向散射光强度(RBC-P70Fsc)分别为(58.5±11.5)ch和(113.3±9.3)ch(P<0.01)、未溶解的红细胞比例(Non- LysedRBC%)分别为(65.7±26.8)%和(85.6±10.6)%(P<0.01)、红细胞平均散射光强度(RBC-MFsc)分别为 (56.1±13.9)ch和(92.8±6.3)ch(P<0.01)、红细胞前向散射光分布宽度(RBC-Fsc-DW)分别为(30.7±9.1)ch和 (19.6±6.8)ch(P<0.05);仪器提示红细胞信息与普通光镜红细胞形态检查结果判断肾小球性血尿的敏感性分别为 91.1%和81.0%,特异性分别为83.6%和85.2%。结论:流式尿沉渣分析仪检测尿红细胞激光参数敏感准确、客观可 靠,是一种有价值的鉴别肾性和非肾性血尿的过筛试验。Objective:To evaluate the clinical significance of the Laser Parameters of RBC in urine for the diagnosis of the source of hematuria.Methods:Hematuria samples from 79 patients with glomerular diseases (diagnosed by renal biopsy) and 128 patients with non-glomerular diseases were examined by UF-100 flow urinary sediment analyzer to get all kinds of laser parameters, which were compared with the morphological results of RBC observed by light microscope.Results:The RBC-P70Fsc, Non-Lysed RBC%, RBC-MFsc and RBC-Fsc-DW of hematuria with glomerular diseases and non-glomerular diseases were respectively (58.5±11.5) and (113.3±9.3)(P<0.01),(65.7±26.8)% and (85.6±10.6)%(P<0.01),(56.1±13.9) and (92.8±6.3)(P<0.01),(30.7±9.1) and (19.6±6.8)(P<0.05).The sensitivity and speciality of UF-100 urinary sediment analyzer were 91.1% and 83.6% respectively,while those of light microscope were 81.0%,and 85.2%.Conclusion:Flow urine analysis by UF-100 flow urinary sediment analyzer is a highly sensitive,easy and reliable method to differentiate the hematuria of glomerular diseases from that of non-glomerular diseases.
