活性同种带瓣补片重建右室流出道研究

摘　　要：目的　探讨液氮冻存活性同种带瓣补片重建右室流出道后对右心功能恢复的影响。方法　22 例复杂先天性心脏病人行RVOT重建术,按跨环补片方法加宽右室流出道及肺动脉,实验组(11例)采用同种带瓣补片,对照组(11例)采用心包补片,术毕测中心静脉压,术后用超声心动图测右心功能主要参数等指标。结果　两组手术均无死亡,主要临床资料两组间比较差别有高度显著性 (P<0.01),实验组优于对照组。结论　采用液氮冻存的活性同种带瓣补片跨肺动脉瓣环加宽右室流出道及肺动脉,可取得满意的防止肺动脉瓣反流效果,术后早期右心功能恢复良好。Objective To evaluate the protection of cryopreserved viable conduit valved aortic homograft patch (CVAH) to right heart function in right ventricular outflow tract(RVOT) reconstruction. Methods 22 patients with complex congenital heart disease were treated RVOT.The diagnosis included double outlet right ventricle and tetralogy of Fallot. They were randomly divided into two groups: experimental group(11 cases) and control group(11 cases),The experimental group were enlarged RVOT over pulmonary ring with cryopreserved viable CAVH patch and the control group with autologous pericardium patch. The central venous pressure was measured as soon as the operation was completed, and the right heart function was measured with ultrasonic cardiography a week later, three months later and six months later. The results of the two groups were compared. Results The two groups have no died case .The main results of the two groups have significant difference (P<0.01).The experiment group were superior to the control group. Conclusion Compared with the RVOT reconstruction with autologous pericardium patch ,the RVOT reconstruction with CVAH patch can effectively avoid pulmonary regurgitation to acquire a better protection to right ventricle function
