摘  要:目的 探讨彩色多普勒超声与肾功能临床相关指标对小儿肾积水的诊断价值。方法 收集2 2例肾积水患儿及2 0例正常小儿肾脏,彩色多普勒超声测量肾门肾动脉主干(MRA)及肾实质内叶间动脉(IRA)的收缩期最大血流速度(Vmax)、舒张期最小血流速度(Vmin)、阻力指数(RI)、搏动指数(PI) ,并比较各参数在各组间的差异。并与核素动态显像测定肾小球滤过率(GFR)及临床常规血检的血清尿素氮(BUN)、血清肌酐(SCr)进行联合分析。结果 小儿肾积水肾脏Vmax、Vmin、RI与正常肾脏各参数比较有显著意义(P<0 .0 5 )。中、重度肾积水的肾功能临床相关指标(BUN、SCr、GRF)与正常组肾功能指标有显著意义(P<0 .0 5 ) ,轻度肾积水的肾功能与正常肾脏在统计学上无显著性差别(P>0 .0 5 )。结论 临床上如能将CDFI的各项参数,尤其RI>0 .7和肾功能临床相关指标相结合将为临床早期治疗、防止肾功能下降提供有价值的诊断。Objective To evaluate the value of color Doppler flow imaging (CDFI)and renal function indices in the diagnosis of children with hydronephrosis.Methods Twenty-two children with hydronephrosis and twenty nor mal children were enrolled.Systolic peak velocity (Vmax),diastolic velocity (Vmin),resistive index(RI) and pulsatility index(PI)of kidneys were measured with CDFI and the parameters were analysed.99mTc-DTPA renal dynamic imaging,glomerular filtration rate(GFR),blood urea nitrogen(BUN),and serum creatinine(SCr) were all simultaneously measured.Results We found that there were significant differences in intrarenal hemodynamics between patients with hydronephrosis and normal children(P<0.05).The renal function was decreased significantly in the moderate and severe hydronaphrosis as compared with the normal group (P<0.05),and no significant difference between the mild group and the normal one(P>0.05).Conclusions We should closely integrate the hemodynamics of CDFI,especially RI>0.7 with clinical renal function indices,in order to treat with surgery early in time and to prevent renal function deteriorated.
