摘  要:目的 比较新型肺通气显像剂锝气体 (Technegas)与习用的99mTc DTPA气溶胶用作肺通气的制备方法和图像质量。方法  4 1例患者吸入Technegas特制发生器生成的锝气体。另 35例患者吸入Cis超声雾化器生成的99mTc DTPA气溶胶。两组患者均采集前位、后位、左侧位、右侧位、左前斜位、右前斜位、左后斜位及右后斜位等 8个体位静态图像。每帧图像采集计数 5 0 0K。比较两组图像质量 ,并分别定量计算两种显像剂的外周肺渗透指数 (PI)。结果 99mTc DTPA气溶胶发生大气道沉积的频率多于Technegas(χ2 =8.5 2 ,P <0 .0 0 5 ) ,且99mTc DTP气溶胶更多形成中央“热区”(P =0 .0 1 )。99mTc DTPA气溶胶组左肺PI0 .86± 0 .1 1 ,右肺 0 .90±0 .1 2 ,平均 0 .88± 0 .0 9。Technegas组左肺PI 0 .92± 0 .1 3,右肺 0 .96± 0 .1 2 ,平均 0 .94± 0 .1 0 ,均高于99mTc DTPA气溶胶组 (t=2 .1 4 ,P <0 .0 5 ;t=2 .37,P <0 .0 5 ;t =2 .78,P <0 .0 1 )。 5 1 %(2 1 / 2 4 )Technegas组患者及 5 7%(2 0 / 35 ) 99mTc DTPA气溶胶组患者出现不同程度消化道放射性沉积现象 ,两者无明显差别 (P >0 .5 )。结论 Technegas较99mTc DTPA气溶胶中央气道沉积少 ,外周肺组织显影好 ,是一种制备方便、安全、有效的肺通气显像剂。Purpose To compare the preparation and image quality obtained with the new radiopharmaceutial Technegas and that of 99mTc-DTPA aerosol in ventilation studies. Methods For Technegas scintigraphy,41 patients inhaled Technegas produced by Technegas Generator.For 99mTc-DTPA aesorol scintigraphy,the other 35 patients inhaled 99mTc-DTPA aesorol produced by Cis ultrasonic nubulizar.Eight views (anterior,posterior,left and right lateral,left and right anterior oblique,left and right posterior oblique) were recorded with a presetting of 500 K counts.Both sets of images were blindly compared for (i) qualitative evaluation of images and (ii) quantitation of peripheral penetration index (PI). Results The use of Technegas led to fewer and less intense foci of bronchial than 99mTc-DTPA aesorol.The difference between Technegas and 99mTc-DTPA aesorol,respectively,between left lung PI (0.92±0.13 vs 0.86± 0.11),right lung PI (0.96±0.12 vs 0.90±0.12) and mean PI (0.94±0.10 vs 0.88±0.09) were significant (P<0.05,P<0.05,P<0.01).Parasitic gastric activity was observed in 51% (21/24) with Technegas and 57% (20/35) with 99mTc-DTPA aesorol,there was not significant difference between the two agents (P>0.5). Conclusions The use of Technegas led to much fewer and less intense foci of bronchial,and it assures good peripheral penetration and alveolar deposition.Technegas inhalation study is convenient,safe and useful method for ventilation study.
