巨大补片加强内脏囊技术在单侧复杂性腹股沟复发疝中的应用

摘　　要：目的:探讨巨大补片加强内脏囊技术(GPRVS)在单侧复杂性腹股沟复发疝中的应用价值。方法:回顾性分析自2001年6月至2004年6月间10例用GPRVS技术治疗单侧复杂性腹股沟复发疝的经验。结果:本组10例均为多次复发疝,其中已行2次手术6例,3次手术4例。术中明确斜疝3例,直疝2例,斜、直复合疝4例,斜、直、股复合疝1例。有8例于术中取出原修补补片。术后10例均放置闭式引流。术后10例均得到随访,随访率100%,随访时间10～38个月,平均27.6个月,无复发发生。10例病人中术后发生尿潴溜1例,阴囊水肿2例,轻微异物感2例。结论:GPRVS技术可以作为治疗单侧复杂性腹股沟复发疝的一种有效方法,在目前聚酯类补片尚未普及情况下,应用聚丙烯材料也不失为一可选之计。Objective To discuss the value of giant prosthetic reinforcement of the visceral sac (GPRVS) for complex unilateral and recurrent inguinal hernias. Methods A retrospective study was made to analyze the experience of using GPRVS in treating complex unilateral and recurrent inguinal hernias in 10 patients between Jun. 2001 to Jun. 2004. Results All the 10 patients had more than one-recurrence of their inguinal hernias; 6 patients had 2 previous operations and 4 had 3 operations. There were 3 indirect hernias, 2 direct hernias, 4 indirect-direct compound hernias and 1 indirect-direct-femoral compound hernia in the series. Eight patients had their originally placed mesh removed during GPRVS. The use of closed drainage was used unexceptionally after GPRVS. The 10 patients were followed up for 10 to 38 months(average 27.6 months), the follow-up rate was 100%. No recurrence occurred after GPRVS. There were 1 case of retention of urine, 2 cases of hydrocele of the scrotum and 2 cases of feeling of foreign body after GPRVS. Conclusions GPRVS could be used as an effective means to treat complex unilateral and recurrent inguinal hernias. During GPRVS, polypropylene mesh could be used instead when polyethylene terephtalate mesh is not available.
