摘 要:目的:观察罗格列酮对2型糖尿病(type 2diabetesmellitus ,T2DM)患者胰岛素治疗时的胰岛素用量,及其对血清尿酸水平、白细胞计数的影响。方法:磺脲类药物继发失效的T2DM患者80例,分为罗格列酮联合胰岛素治疗组(A组)和单纯胰岛素治疗组(B组)。观察两组治疗前后空腹血糖、餐后2h血糖、血尿酸水平、白细胞计数,及治疗12周后全天胰岛素用量。结果:A组和B组能同样有效地降低血糖,但A组胰岛素用量低于B组(2 2 .9±5 .3)U比(2 6 .4±6 .8)U ,P <0 .0 5 ,治疗后A组尿酸水平显著下降,由治疗前的(2 71.4±10 7.2 ) μmol/L降为(2 2 3.1±78.5 ) μmol/L ,P <0 .0 5 ,同时其白细胞计数的对数值亦显著降低(9.6 3±0 .0 9)比(9.72±0 .13) ,P <0 .0 1。结论:罗格列酮能减少T2DM患者胰岛素治疗时胰岛素用量,同时降低其血清尿酸水平和白细胞计数;提示罗格列酮在减少T2DM患者胰岛素使用量的同时,对体内炎性反应有一定的改善作用。Objective:The dosage of insulin, leve l of serum uric acid and the cou nt of white blood cell were observed in patients with type 2 diabetes mellitus ( T2DM) treated with insulin combined with Rosiglitazone. Methods: Eighty patients with the secondary failure to sulfonylureas were divided into insulin combined with Rosiglitazone treatmen t group and insulin treatment group. The whole observation lasted for 12 weeks. Fasting blood glucose (FBG), postprandial blood glucose (PBG), serum uric acid a nd white blood cell (WBC) were analyzed pre-treatment and post-treatment. Results:Similar decreases in FBG and PBG were found in bo th two groups, while the dosages of insulin in insulin combination group were s ignificantly lower than that in insulin alone group(22.9±5.3 vs 26.4±6.8 , P<0.05). After 12 weeks treatment, the level of serum u ric acid was significantly decreased in insulin combination group (pre-treatment 271.4±107.2 , post-treatment 223.1±78.5 , P<0.0 5 ) and the logarithm of the count of white blood cell was also significantly de creased (pre-treatment 9.72±0.13, post-treatment 9.63±0.09, P<0.01 ) Conclu sion:While Rosiglitazone can decrease the dosage of insulin, it can also decrease both the serum le vel of uric acid and the count of white blood cell, which indicates it can impro ve the inflammatory reactions in patients with T2DM .
