当归补血汤加味治疗氯氮平所致白细胞减少症的对照研究

摘　　要：目的:观察当归补血汤加味对氯氮平所致白细胞减少症的作用,并探讨其作用机制。方法:将6 0例氯氮平所致白细胞减少症的病人随机分为当归补血汤加味治疗组30例;鲨肝醇、Vit.B4 对照组30例,共治疗6周,每2周查白细胞1次,比较其结果。结果:治疗组显效18例,有效10例,无效2例,总有效率为93.3% (95 % CI=77.9%～99.2 % ) ;对照组显效12例,有效12例,无效6例,总有效率为80 .0 % (95 % CI=6 1.4 %～92 .3% )。两组综合疗效相当(u =1.6 10 0 ,P =0 .10 75 )。治疗4周后,治疗组升高白细胞参数优于对照组(P <0 .0 0 1)。结论:当归补血汤加味治疗氯氮平所致的白细胞减少症疗程短、疗效好,综合疗效与鲨肝醇、Vit.B4 相当,其收益NNT=8,治疗4周后治疗组升高白细胞作用优于对照组。Objective:To observe the effect of DBD in treatin g leukopenia induced by clozapine and to study the mechanism.Method:60 cases of le ukopenia were randomly divided into the treatment group treated with DBD(30 cases) and the control group treated with batilol and Vit.B 4 (30 cases) for 6 weeks.Blood leukocyte of the two groups was detected every 2 weeks.Result:In th e treatment group,18 cases significantly effective, 10 cases effective,2 caese i neffective,the total effective rate was 93.3%(95%CI=77.9%～99.2%).In the control group,12 cases significantly effective,12 cases effective,6 cases ineffective, the total effective rate was 80.0%(95%CI=61.4%～92.3%).The difference between th e two group wasn't notable(u=1.6100,P=0.1075).4 weeks after treatment,the p arameters of leukocyte in the treatment group were superior to that in the contr ol group(P<0.001).Conclusion:The total effect of DBD in treating leu kopenia induced by clozapine was equal to that of batilol and Vit.B 4.NNT=8.B ut after 4 weeks,the number of leukocyte in the treatment group was higher than that in the control group.
