^(13)C-UBT和胃镜法诊断胃幽门螺杆菌感染的对比研究

摘　　要：目的　对比研究13 C -UBT和胃镜法对胃幽门螺杆菌(Helicobacterpylori,Hp)感染的诊断价值.方法　分析了2 0 4例Hp感染患者均行13 C -UBT和胃镜(活检组织切片法)检查.比较两种方法的阳性率及计算13C -UBT的敏感性、特异性、阳性预测值和阴性预测值.结果　13 C -UBT阳性率为4 4 .6 % ,胃镜法的阳性率为4 2 .2 % ,两者无差异(p>0 .0 5 ) .13 C -UBT的敏感性为98.8% ,特异性为94 .9% ,阳性预测值为93.4 % ,阴性预测值为99.1% .结论　与胃镜法相比,13 C -UBT诊断Hp方法简单、安全、无创,具有较高的敏感性和特异性,可作为Hp诊断的首选方法.Objective To compare the value of13C-urea breath te st (13C-UBT) between stomach endoscopic biopsy for the diagnosis of helico bacter pylori's infection. Methods To retrospectively analyzed 204 patients in our hospital from Jan 2003 to Jun 2004. all these patients accep ted13C-UBT and stomach endoscopic biopsy.We compare the positive rate the se two methods,and calculated the sensitivity, the specificity, the positive pr edictive value,the negative predictive value of13C-UBT. Results The positive rate of13C-UBT was 44.6%, the stomach endoscopic biop sy 42.2%, there were no statistical difference between them. The sensitivity of13C-UBT was 98.8%,the specificity 94.9%, the positive predictive value 93. 4%, the negative predictive value 99.1%. Conclusions Compared w ith stomach endoscopic biopsy,13C-UBT is a simple, safe and noninvasive me thod with more higher sensitivity and specificity, and may be the first choice f or the diagnosis of helicobacter pylori's infection.
