摘　　要：目的评估β受体拮抗剂对多巴酚丁胺负荷超声心动图(DSE)检测存活心肌的影响。方法选择冠状动脉旁路移植术(CABG)前左室射血分数≤40%患者26例,术前行DSE检测存活心肌。将DSE前未停用β受体拮抗剂的患者设为A组,余设为B组。多巴酚丁胺分5滋g/kg·min和10滋g/kg·min两种剂量级。CABG术后约3个月随诊检查。结果A组9例、B组17例患者临床特点比较无明显差异,但A组患者平均冠状动脉狭窄率略高于B组。5滋g/kg·minDSE检测存活心肌的敏感性降低(A组51.5%,B组67.9%),而10滋g/kg·minDSE的结果所受明显影响较小(A组72.5%,B组74.4%),无统计学差异。结论对于严重冠状动脉疾病合并左心室功能不全的患者,口服小剂量β受体拮抗剂可能降低5滋g/kg·minDSE检测存活心肌的敏感性,而对10滋g/kg·minDSE的影响明显减小。Objective To evaluate the effects of B-blocker on dobutamine stress echocardiography (DSE) in detection of myocardial viability. Methods Patients who were scheduled to undergo coronary artery bypass grafting(CABG) with left ventricular dysfunction ( left ventricular ejection fraction <40%= were screened enrollmen in the study. Each patient underwent a DSE within a week before CABG was followed with echocardiography 3 months after CABG . Results 9 patients who still on B-blockers when accepted DSE was A group,the others were B group.There were not difference between two groups in clinical charicterization included old ,sex, wall motion score index , clinical heart function but severity of coronary artery stenosis .There were decrease of sensitivity of DSE in the detection of myocardial viability in A group when 5ug/kg/min DSE( 51.5% , 67.9% respectively) but no signification difference in tow group when 10ug /kg /min(72.5%, 74.4% respectively). Conlusions B-blocker maybe decrease the sensitive of dobutamine stress echocardiography to detect viable myocardum at 5μg / kg /min dose,but not markedly attenuates the same ability of at 10μg /kg /min dose testing.
