摘 要:目的 探讨CT值测量差值识别超急性期脑梗死的准确性。方法 将临床追踪证实的34例超急性期脑梗死组与31例神经系统正常志愿者的头颅CT平片按有无CT值标注分为2组,分别进行专家组、青年医师组双盲读片,对照分析,根据CT征象作出诊断,并比较2组的诊断准确率。结果 青年组应用CT值测量差值前后诊断准确率分别为29. 41%、91. 18%;专家组分别为41. 18%及94. 10%。统计学显示差异有高度显著性。结论 应用CT值测量对称部位脑实质密度差值(ΔHU)来识别低密度征的存在,可以让医师更有效地识别脑梗死灶的有无,特别是对缺乏经验的医师帮助更大,从而提高超急性期脑梗死的诊断准确率。Objective To study the accuracy of the routine plain CT ΔHU in diagnosing super acute cerebral infarction.Methods 31 volunteers with normal neuro system and 34 patients with confirmed super acute cerebral infarction were undertaken routine plain cerebral CT scan. The two CT film (one with CT value and one without CT value )of each case were analyzed and diagnosed by expert group and young doctor group respectively. The accuracy rates of both groups were compared. Results The accuracy rates before and after applying ΔHU of routine plain CT scan to diagnose cerebral infarction among the young doctor group were 29.41% and 91.18% respectively, and 41.18% and 94.10% among expert group. There was significant difference. Conclusion The application of ΔHU of CT scanning symetric cerebral structure to identify the signs of low density can help the doctors to find nidus of cerebral infarction, especially for the doctors lack of experience, which finally promotes the accuracy in diagnosing super acute cerebral infarction.
