针灸药结合疗法对慢性乙型肝炎患者肝组织病理学的影响

摘　　要：目的:探讨针灸药结合治疗(简称三环疗法)对慢性乙型肝炎(慢乙肝)患者肝组织病理学的影响。方法:随机选择3 0例慢乙肝患者采用三环疗法进行治疗。治疗前后分别进行肝组织活检、血清肝纤维化指标检测。结果:4疗程后肝组织病理学情况:1例由G4S4降为G3 S3 ,7例由G4S4降为G2 S2 ,9例由G3 S3 降为G2 S2 ,5例由G3 S3降为G1S1,8例由G2 S2 降为G1S1。治疗前后炎症活动计分分别为9 . 2 3±3 . 8和4 .85±2 . 6;纤维化程度计分分别为8 .78±6 . 5和5 .70±4 . 3 ,两项治疗前后比较差异均有显著性意义(P <0 .0 5 )。血清肝纤维化指标:治疗前后血清透明质酸(HA)分别为(689 . 4±71. 5 )ng/ml和(2 17 . 6±5 7. 2 )ng/ml,血清层粘蛋白(LN)分别为(2 41 .3±67. 5 )ng/ml和(14 1. 7±3 .7 4)ng/ml ,两项指标治疗前后比较差异有非常显著性意义(P <0. 0 1)。结论:针灸药结合治疗对慢乙肝患者肝组织病理学有明显的改善作用。Objective:Analysis of therapeutic effect of the liver histological improvement in patients with chronic hepatitis B by the combined treatment of acuquncture with moxibustion and medicinal herbs(brief name:Three-cycle Therapy ). Methods:30 CHB patients were randomly enrolled and received Three-cycle Therapy for 4 courses of treatment.Liver samples were taken from the patients before and after treatment and evaluated the degree of inflammation and fibrosis.The serum fibrosis markers were detected too.Results:The liver histology of 30 patients had all been improved on different degree after therapy 1 case liver histology had been better from G 4S 4→G 3S 3,7 cases from G 4S 4→G 2S 2,9 cases from G 3S 3→G 2S 2,5 cases from G 3S 3→G 1S 1,8 cases from G 2S 2→G 1S 1.After therapy the scores of inflammation and fibrosis degree were also decreased from 9.23±3.8 to 4.85±2.6( P <0.01) and 8.78±6.5 to 5.70±4.3( P <0.05),The serum fibrosis markers hyaluronic acid(HA) and laminin(LN) from (689.4±71.5)ng/ml to (217.6±57.2)ng/ml( P <0.01)and (241.3±67.5)ng/ml to (141.7±37.4)ng/ml( P <0.01)?Conclusion:Acupuncture combined with moxibustion and medicinal herbs can obviously improve the liver histology of chronic hepatitis B.
