摘  要:目的 探讨呼吸阻抗测定在慢性阻塞性肺疾病(COPD)中的价值。方法 采用脉冲振荡法(IOS)对62例COPD患者进行呼吸阻抗测定并将测定结果与常规肺通气功能测定结果比较。结果 COPD患者黏性阻力较健康组明显增高,COPD组呼吸阻抗指标中Fres、R5、R20、R5 R20与FEV1 /FVC、FEV1 (%pred)、Vmax50 (%pred)、Vmax25 (%pred)、MMEF(%pred)均呈显著负相关,而X5 则与它们呈正相关,Fres与FEV1 (%pred)的相关性最强,Fres诊断COPD的敏感性最高,其次为R5、X5。结论 IOS法测定COPD患者呼吸阻抗与常规肺通气功能测定有显著的相关性,具有临床应用前景。Objective To study the value of total respiratory impedance in chronic obstructive pulmonary disease(COPD). Methods The impedance of respiratory system was measured by impulse oscillometry system(IOS) and compared with parameter values of routine pulmonary ventilation function in 62 patients with COPD. Results There was a greater increase for viscous resistance (R) in COPD group than in health group. Fres, R 5, R 20 , R 5-R 20 were negatively correlated with FEV 1/FVC, FEV1(%red), Vmax 50 (%pred) , Vmax 25 (%pred), MMEF(%pred); X 5 was positively correlated with them. A closest correlation was found between Fres and FEV1(%pred). The sensitivity of Fres in diagnosis of COPD was the highest, followed by that of R 5 and X 5. Conclusion The significant correlation exists between total respiratory impedance by IOS and parameters of routine pulmonary ventilation function. The future development of IOS could be prosperous.
