摘  要:目的从激素和雌雄激素受体变化探讨隐睾症发病机制.方法化学发光免疫技术检测30例隐睾症和16例斜疝(对照组)患儿血清促卵泡生成激素(FSH)、促黄体生成激素(LH)和睾丸酮(T)激素水平;免疫组化技术观察隐睾症患儿睾丸引带、阴囊肉膜、阴囊皮肤和斜疝患儿疝囊组织雄激素受体(AR)和雌激素受体(ER)表达情况.结果①激素水平:隐睾症组血清LH和T水平与斜疝组二者之间无显著性差异(P<0.05);双侧隐睾组血清FSH水平高于斜疝组(0.01<P<0.05),单侧隐睾症组血清FSH与斜疝组差别无统计学上意义(P<0.05). ②AR和ER表达:隐睾症组睾丸引带AR基本不表达,阴囊肉膜和阴囊皮肤呈低表达,ER为中~高表达.斜疝组疝囊组织AR呈高~中表达,ER基本不表达.结论睾丸引带AR低表达和ER高表达可能是造成睾丸下降不全的原因之一.Objective To assess the expressions of androgen receptor (AR)and estrogen receptor (ER) in cryptorchidism.Methods Serum levels of FSH, LH and Testosterone were studied by fluoresent immunocytometry in 30 patients with cryptorchidism and 16 patients with indirect inguinal hernia (Control group). Expressions of AR and ER in gubernacular ligament, scrotal dartos, scrotal skin and tissue of hernia sac were examined with immunohistochemistry.Results There was no difference in LH and T levels between cryptorchidism and the control groups(P< 0.05). The serum FSH level of patients with bilateral cryptorchidism was significantly higher than that of the control group ( 0.01<P< 0.05). There was no difference between unilateral cryptorchidism and the control group (P< 0.05). AR expression was absent in gubernacular ligament, lower in scrotal tissue whereas ER expression was higher in cryptorchidism. AR expression was up-regulated and ER expression was absent in hernia sac.Conclusions Loss of expression of AR and higher expression of ER is associated with altered gubernacular development and cryptorchidism.
