超声心动图对房颤电转复后的心房机械功能的研究

摘　　要：目的　应用超声心动图观察房颤患者电除颤后心房收缩功能的变化。方法　2 0例房颤患者成功电除颤后,于复律后1 d、3 d、7d、1个月分别行超声心动图检查,测量二尖瓣血流频谱A峰流速( VA)、A峰速度时间积分( A- VTI)、心房充盈分数( AFF)和心房射血力( AEF) ,将结果进行统计学分析。结果　恢复窦律后第1 d,VA、A- VTI、AFF和AEF均明显低于正常组,随时间推移逐渐增高,VA、A- VTI、AFF至第7d时基本恢复到正常水平,AEF至除颤后1个月时基本恢复正常。结论　房颤转复后心房收缩功能出现一过性减低;VA、A- VTI。Objective To evaluate atrial mechanical function after cardioversion of atrial fibrillation.Methods A total of 20 atrial fibrillation patients treated with direct-current shock successfully.They all underwent echocardiographic examination 1,3,7 days and 1 month after the restoration of sinus rhythm and month.We also had 20 normal individuals for comparision.The following parameters were evaluated:peak A velocity(VA),velocity time integral of A-wave(A-VTI),atrial filling fraction(AFF),and atrial ejection force(AEF).Results VA,A-VTI,AFFand AEF were decreased compared with that of the contral group 1 day after cardioversion,but they all had a trend toward higher along with the time.VA,A-VTI and AFF came to normal criteria 7 days after cardioversion,and AEF came to normal until 1 month.Conclusions There is a transient atrial mechanical dysfunction after cardioversion of atrial fibrillation;VA?A-VTI,AFFand AEF can be used to evaluate atrial systolic function.
