摘  要:目的探讨质子泵拮抗剂对14 C UBT的影响及其校正方法。方法49例幽门螺杆菌阳性的病人随机分成2组,分别口服泮托拉唑40mg和雷贝拉唑10mg共14天。于用药第14天测定14 CO2 DPM值,HP阴性患者口服柠檬酸液2 0 0mL(含4g柠檬酸)后再次测14 CO2 DPM值。结果泮托拉唑组和雷贝拉唑组分别有2 5 9% ( 7/2 7)及3 1 8% ( 7/2 2 )出现阴性,且两组14 CO2 DPM值较前明显下降(P <0 0 5 ) ,但两组比较无显著性差异。服用柠檬酸后13 /14例HP阳性,但其14 CO2 DPM值仍低于服药前(P <0 .0 5 )。结论质子泵拮抗剂对14 C UBT的结果有明显的影响,但可通过口服柠檬酸的方法得以校正。Objective To investigate the effect of proton pump inhibitors on ~14 C_urea breath test (~14 C_UBT) and its emendatory methods. Methods Forty_nine patients who were prospectively evaluated positive for H. Pylori by ~14 C-UBT, were randomized to receive either pantoprazole 40 mg/day or rabeprazole 10 mg/day for 14 days. A repeat breath test was performed on a day 14 to evaluate the ~14 CO_2 DPM.If the patients were negative for HP, they were given a drink of 200 mL containing 4 g citric acid and then determined the ~14 CO_2 DPM by ~14 C-UBT. Results Pantoprazole group and rabaprozole group had negative rates of 25.9%(7/27) and 31.8%(7/22) for HP test. ~14 CO_2 DPM of two groups decreased significantly compared with those before receiving drugs (P<0.05), but had no significant difference between two drug groups(P<0.05). In 14 patients with nagetive HP, 13/14 cases were HP positive after receivd citric acid, but ~14 CO_2 DPM was still below that before receving medicine (P<0.05).Conclusion Proton pump inhibitors significantly affected the accuracy of the UBT and caused false nagetive results but could be emended by recieiving citric acid.
