摘  要:目的 观察系统性红斑狼疮 (systemiclupuserythematosus ,SLE)病人外周血CD4 +及CD8+T细胞表面可诱导共刺激分子 (inducibleco stimulator ,ICOS)及CD2 8共表达水平 ,探讨ICOS和CD2 8共表达水平与SLE疾病的关系。方法 采用三色流式细胞术检测SLE病人 (n =51 )及正常人 (n =30 )外周血CD4 +及CD8+T细胞表面ICOS与CD2 8共表达水平 ,并结合SLE病人疾病活动程度、临床表现等进行分析。结果 与正常人相比 ,SLE活动期和稳定期病人外周血CD4 +T细胞中仅表达ICOS不表达CD2 8(即CD2 8- ICOS+)的细胞比例明显升高 ,但活动期病人和稳定期病人之间并无差异 ;活动期病人外周血CD8+T细胞上同时表达CD2 8和ICOS的细胞 (即CD2 8+ICOS+细胞 )和CD2 8- ICOS+细胞比例都明显升高 ;同一SLE病人在疾病活动期CD4 +和CD8+T细胞中CD2 8+ICOS+的细胞比例和CD8+T细胞中CD2 8- ICOS+细胞比例明显高于该病人经过治疗病情稳定时 ;初发未治疗SLE病人仅CD4 +T细胞中CD2 8+ICOS+细胞的比例比复发病人明显升高 ;血清抗dsD NA抗体 ( + )病人和血清免疫球蛋白含量 (IgG、IgA和IgM中任何一种 )异常升高的病人外周血CD4 +T细胞中CD2 8+ICOS+细胞和CD8+T细胞中CD2 8- ICOS+细胞比例分别明显高于血清抗dsDNA抗体 ( )和血清免疫球蛋白含量正?Objective To investigate the level and clinical significance of inducible co stimulator (ICOS) and CD28 molecules co expression on peripheral CD4 + and CD8 + T lymphocytes in patients with systemic lupus erythematosus(SLE). Methods The levels of ICOS and CD28 co expression on peripheral CD4 + and CD8 + T lymphocytes in 51 patients with SLE and 30 normal individuals were detected by three color flow cytometry and the results were analyzed along with disease activity, disease stage, level of anti dsDNA antibody and immunoglobulin in serum of SLE patients. Results Compared with normal individuals, the percentages of CD28 - ICOS + cells on the peripheral CD4 + T cells in patients with SLE during active and stable stages were significantly increased, but there was no significant difference between the two group patients. The percentages of CD28 +ICOS + and CD28 -ICOS + cells on the peripheral CD8 + T cells of SLE patients during active stage were significantly increased; In the same patients, the percentages of CD28 +ICOS + cells on peripheral CD4 + and CD8 + T cells and the percentages of CD28 -ICOS + cells on CD8 + T cells during active disease stage were significantly increased compared with those in stable disease stage; the percentage of CD28 +ICOS + cells on the peripheral blood CD4 + T cells in untreated primary patients was higher than those with disease relapse; the percentages of CD28 +ICOS + cells on the peripheral CD4 + T cells and CD28 -ICOS + cells on CD8 + T cells were significantly increased in patients with serum anti dsDNA antibody(+) and patients with aberrantly high content of immunoglobulin compared with those with serum anti dsDNA antibody( ) and patients with normal content of immunoglobulin, respectively. Conclusion The level of ICOS and CD28 co expression on the peripheral blood CD4 + and CD8 + T cells in patients with SLE had potential relationship with disease activity, disease stages and clinical manifestations.
