淮北地区产ESBLs菌株的分布特征

摘　　要：目的　了解淮北地区三家医院产超广谱β内酰胺酶(ESBLs)菌株的分布情况,以利于对ELBLs菌株的监控和临床治疗。方法　采用1999年NCCLS推荐的初筛试验,从2 2 3株大肠埃希菌和肺炎克雷伯菌中筛选出可疑产ESBLs细菌,并用确证试验加以确认。结果　176株大肠埃希菌和4 7株肺炎克雷伯菌中共检出10 2株产ESBLs(45 .7% ) ,其中大肠埃希菌74株(42 .0 % ) ,肺炎克雷伯菌2 8株(5 9.6 % )。ESBLs检出率最高的标本是呼吸道分泌物(44 .2 % ) ;各病区产ESBLs菌的分离率以重症监护(ICU)病房最高,为73.7% ;其次是干部病房6 5 .4 % ,呼吸科病房为4 5 .2 % ,其他病房为34.4 %。结论　淮北地区ESBLs的检出率为4 5 .7% ,ICU病房和呼吸道分泌物是ESBLs的主要来源。Objective To study the distribution of clinical isolates which produced extended spectrum β-lactamases(ESBLs) from three hospitals in Huaibei area of Anhui province for better control and treatment.Methods 223 strains of K. pneumoniae and E. coli conserved were determined with screening disc and confirmation method by 1999 NCCLS guideline.Results The positive rate of ELBLs-producing among K.pneumoniae and E.coli was 45.7%. And 44.2% of all ESBLs strains were isolated from respiratory tract secretions. The isolated rates of ESBLs were 73.7% in ICU wards,65.4% in veterans wards, and 34.4% in others respectively.Conclusion The positive rate of ESBLs-producing among K.pneumoniae and E.coli is 45.7%,the main sources of producing ESBLs bacteria are ICU ward and respiratory tract secretions.
